摘要
目的 回顾性分析复旦大学附属中山医院厦门医院(我院)2019—2021年住院药房麻醉药品的使用情况及其变化趋势,为麻醉药品在我院的科学管理和合理使用提供参考。方法 从我院HIS电子病历系统导出2019—2021年麻醉药品相关使用数据,对销售金额、用药频度(DDDs)、日均费用(DDC)以及排序比(B/A)进行统计分析。结果 我院住院药房的麻醉药品包括14种品规,年销售比例呈逐年上升趋势。其中注射用盐酸瑞芬太尼(1 mg)、芬太尼透皮贴剂、盐酸羟考酮缓释片(10 mg)的销售金额及排序在3年中均稳居前3位;3年来麻醉药品DDDs排序在前三的分别是芬太尼透皮贴剂(4.2 mg)、盐酸羟考酮缓释片(10 mg)和注射用盐酸瑞芬太尼(1 mg);注射用盐酸瑞芬太尼(1 mg)的DDC稳居第1位。结论 我院麻醉口服片剂、芬太尼透皮贴剂及注射用盐酸瑞芬太尼使用较频繁,麻醉药品的使用及管理基本符合相关标准和诊疗规范,但仍存在部分不足,今后仍需规范临床用药,加强监控与干预。
Objective To retrospectively analyze the use of anesthetic drugs and its changing trends in inpatient pharmacies of Zhongshan Hospital, Hehai University(Xiamen Branch)(our hospital) from 2019 to 2021, so as to provide reference for the scientific management and rational use of anesthetic drugs in our hospital. Methods The related usage data of anesthetic drugs from 2019 to 2021 were derived from HIS electronic medical record system of our hospital. Statistical analysis was performed on the sales amount, defined daily dose system(DDDs), defined daily cost(DDC) and ranking ratio(B/A). Results The anesthetic drugs in the inpatient pharmacy of our hospital included 14 kinds of specifications, and the annual sales proportion showed an increasing trend year by year. Remifentanil hydrochloride for injection(1 mg), fentanyl transdermal patch and oxycodone hydrochloride prolonged-release tablet(10 mg) all firmly ranked as the top three anesthetic drugs with the highest sales amounts in the past three years. Fentanyl transdermal patch(4.2 mg), oxycodone hydrochloride prolonged-release tablet(10 mg) and remifentanil hydrochloride for injection(1 mg) ranked as the top three anesthetic drugs with the highest DDDs in the past three years. Remifentanil hydrochloride for injection(1 mg) ranked first in DDC. Conclusion Anesthetic oral tablets, fentanyl transdermal patch and remifentanil hydrochloride for injection are used more frequently in our hospital. The use and management of anesthetic drugs basically meet the relevant standards and diagnostic specifications, but there are still some deficiencies. It is necessary to standardize clinical medication and strengthen monitoring and intervention in the future.
