摘要
目的 探讨合成磁共振T_(1)、T_(2)、质子密度(PD)定量值在诊断髌软骨早期损伤中的效能.方法 采用GE Signa pioneer 3.0 T合成磁共振成像技术对44名健康志愿者及36例髌软骨早期损伤患者进行定量分析,软骨损伤以关节镜结果为金标准,对比分析合成磁共振T_(1)、T_(2)、PD定量值在诊断髌软骨早期损伤中的效能.结果 髌软骨不同分级损伤PD、T_(1)及T_(2)均值存在差异(P<0.05);PD均值组间比较:正常组与Ⅰ级损伤组、Ⅱ级损伤组存在差异(P<0.05),Ⅰ级损伤组与Ⅱ级损伤组差异无统计学意义(P>0.05);T_(1)均值组间比较:正常组与Ⅰ级损伤组、Ⅱ级损伤组存在差异(P<0.05),Ⅰ级损伤组与Ⅱ级损伤组差异无统计学意义(P>0.05);T_(2)均值组间比较:正常组、Ⅰ级损伤组及Ⅱ损伤组各组间均存在差异(P<0.05).T_(2)﹑T_(1)及PD值在诊断髌软骨早期损伤的受试者工作特征曲线(ROC)曲线下面积(AUC)分别为0.871,0.730,0.654,其中T_(2)值诊断的敏感度及特异度分别为80.6%、84.1%.结论 合成磁共振定量值在一定程度上反映软骨早期损伤的程度,T_(2)值效能最高.
Objective To explore the efficacy of synthetic magnetic resonance quantitative values of T_(1),T_(2) and PD in the diagnosis of early patellar cartilage injury.Methods 44 healthy volunteers and 36 patients with early patellar cartilage damage were quantified in GE Sign A Pioneer 3.0 T synthetic MRI.The efficacy of synthetic quantitative values of T_(1),T_(2) and PD in the diagnosis of early patellar cartilage injury were summarized,compared with arthroscopy.Results The mean PD T_(1) and T_(2) values were different among different grades of patellar cartilage injury(P<0.05).There were significant difference between normal group and gradeⅠinjury(P<0.05),normal group and gradeⅡinjury(P<0.05),and was not significant difference between gradeⅠinjury and gradeⅡinjury(P>0.05).There were significant difference normal group and gradeⅠinjury(P<0.05),normal group and gradeⅡinjury(P<0.05),and there was no significant difference between gradeⅠinjury and gradeⅡinjury(P>0.05).There were differences in normal group,gradeⅠand Ⅱinjury groups(P<0.05).The areas of T_(2),T_(1) and PD values under ROC curve for diagnosing early patellar cartilage injury were 0.871,0.730 and 0.654,respectively.The sensitivity and specificity of T_(2) value were 80.6%and 84.1%,respectively.Conclusion The synthetic magnetic resonance quantitative value reflects the degree of cartilage damage to a certain extent,and T_(2) value is the most effective.
