MR transverse relaxation time imaging for differential diagnosis of benign and malignant parotid gland tumors

摘要 目的评价横向弛豫时间成像(T2 mapping)鉴别诊断腮腺良恶性肿瘤的价值。方法回顾性分析73例接受T2 mapping检查并经手术病理证实的腮腺肿瘤患者,其中49例为良性肿瘤(良性组),包括29例多形性腺瘤(多形性腺瘤亚组)、17例沃辛瘤(沃辛瘤亚组)及2例基底细胞腺瘤和1例神经纤维瘤,24例为恶性肿瘤(恶性组);测量并比较组/亚组间T2值差异,采用受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评价T2值鉴别诊断腮腺肿瘤的价值。结果良恶性肿瘤T2值差异有统计学意义(t'=3.24,P<0.01),T2值鉴别腮腺良、恶性肿瘤的AUC为0.69,其诊断敏感度为64.69%,特异度为85.72%。多形性腺瘤亚组、沃辛瘤亚组与恶性组间T2值差异有统计学意义(F=18.49,P<0.01),两两比较示Warthin瘤亚组与恶性组差异无统计学意义(P=0.18),其余组/亚组间差异均有统计学意义(P均<0.05)。T2值鉴别多形性腺瘤与恶性肿瘤的AUC为0.83,敏感度为79.63%,特异度为94.31%;鉴别沃辛瘤与恶性肿瘤的AUC为0.91,敏感度为93.28%,特异度为89.84%。结论T2 mapping对鉴别诊断腮腺良、恶性肿瘤,多形性腺瘤与沃辛瘤和多形性腺瘤与恶性肿瘤具有一定价值。 Objective To explore the value of transverse relaxation time imaging(T2 mapping)for differential diagnosis of benign and malignant parotid gland tumors.Methods Data of 73 patients with parotid gland tumors detected with head and neck MR scanning and confirmed by operation and pathology were retrospectively analyzed,including 49 cases of benign tumors(benign group,with 29 cases of pleomorphic adenoma[pleomorphic adenoma subgroup],17 cases of Warthin tumor[Warthin tumor subgroup],2 cases of basal cell adenoma and 1 case of neurofibroma)and 24 malignant tumors(malignant group).All patients underwent T2 mapping.T2 values of lesions were calculated and compared between groups/subgroups.The receiver operating characteristic(ROC)curve was drawn,and the area under the curve(AUC)was calculated to evaluate the efficacy of T2 value for differential diagnosis of benign and malignant parotid tumors.Results T2 values were statistically different between benign and malignant groups(t′=3.24,P<0.01).AUC of T2 value for differential diagnosis of benign and malignant parotid tumors was 0.69,with sensitivity of 64.69%and specificity of 85.72%.T2 values were significantly different among pleomorphic adenoma subgroup,Warthin tumor subgroup and malignant group(F=18.49,P<0.01).Pairwise comparison showed that there was no significant difference of T2 values between malignant group and Warthin tumor subgroup(P=0.18),while there were significant differences among the other group/subgroups(both P<0.05).AUC of T2 values for differentiating pleomorphic adenoma from malignant parotid tumor was 0.83,with sensitivity of 79.63%and specificity of 94.31%.AUC of T2 value for differentiating Warthin tumor from malignant parotid tumor was 0.91,with sensitivity of 93.28%and specificity of 89.84%.Conclusion T2 mapping had certain value for differential diagnosis of benign and malignant parotid tumors,pleomorphic adenoma and Warthin tumor,also pleomorphic adenoma and malignant tumor.
