摘要
目的 观察护肝利水汤联合中药敷脐对肝硬化腹水患者门静脉压力的影响。方法 将我院130例肝硬化腹水患者随机分为观察组与对照组各65例。对照组采用西医治疗,观察组在对照组治疗基础上采用护肝利水汤联合中药敷脐治疗。比较两组患者临床疗效,两组患者治疗前后体质量、腹围、尿量、腹水深度,胆红素(TBIL)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、白蛋白(ALB)水平,门静脉主干内径、门静脉血流速、脾静脉内径。结果 治疗后,观察组总有效率高于对照组(P<0.05);两组体质量、尿量、ALB水平、门静脉血流速均增加,腹围、腹水深度、TBIL、ALT、AST水平、门静脉主干内径、脾静脉内径均减少(均P<0.05);观察组体质量、尿量、ALB水平、门静脉血流速多于对照组,腹围、腹水深度、TBIL、ALT、AST水平、门静脉主干内径、脾静脉内径低于对照组(P<0.05)。结论 护肝利水汤联合中药敷脐能减少肝硬化腹水患者体质量、腹围,增加尿量,促进腹水消退,提高肝功能,改善门静脉压力,缩小脾静脉内径,疗效确切。
Objective To observe the effect of Hugan Lishui Decoction combined with TCM navel compressing in patients with liver cirrhosis and ascites. Methods A total of 130 patients with liver cirrhosis and ascites were randomly divided into observation group and control group,with 65 cases in each group. Control group was treated with western medicine,and observation group was treated with Hugan Lishui Decoction combined with TCM navel compressing on the basis of control group. The clinical efficacy of the two groups was compared after;the body weight,abdominal circumference,urine volume,ascites depth,bilirubin(TBIL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin(ALB)levels,the internal diameter of the main portal vein,the portal vein blood flow rate,and the internal diameter of the splenic vein were compared between the two groups before and after treatment. Results After treatment,the total effective rate in observation group was higher than that in control group( P<0.05);the body weight,urine volume,ALB level,and portal vein blood flow rate of both groups increased,while the abdominal circumference,the depth of ascites,TBIL,ALT,AST levels,the inner diameter of the main portal vein,the inner diameter of the splenic vein decreased(all P<0.05);the levels of ALB and portal vein blood flow were higher than those in control group,while the abdominal circumference,the depth of ascites,the levels of TBIL,ALT,AST,the inner diameter of the main portal vein,the inner diameter of the splenic vein were lower than those in control group( P<0.05). Conclusion Hugan Lishui Decoction combined with TCM navel compressing can reduce the body weight and abdominal circumference,increase urine volume,promote ascites regression,improve liver function,improve portal vein pressure,reduce splenic vein diameter.
