摘要
目的探讨与分析壮骨止痛胶囊对绝经后骨质疏松椎体压缩骨折经皮穿刺椎体成形术(percutaneous vertebroplasty,PVP)后患者的影响。方法前瞻性选择2019年5月~2022年1月在庆阳市中医医院完成PVP手术的96例骨质疏松椎体压缩骨折的绝经后患者作为研究对象。根据随机数表法将患者分为中药组与对照组,每组48例。对照组给予常规物理治疗,中药组在对照组治疗的基础上给予壮骨止痛胶囊治疗,两组均治疗观察3个月。比较两组治疗前后胸腰背疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)脊柱评分、腰椎椎体骨密度变化及治疗期间并发症发生情况。结果治疗后,两组胸腰背VAS评分均低于治疗前,差异有统计学意义(P<0.05),且中药组明显低于对照组,差异有统计学意义(P<0.05)。治疗后,两组ODI评分均明显低于治疗前,差异有统计学意义(P<0.05),且中药组明显低于对照组,差异有统计学意义(P<0.05)。治疗后,两组腰椎椎体骨密度均明显高于治疗前,差异有统计学意义(P<0.05),且中药组明显高于对照组,差异有统计学意义(P<0.05)。中药组椎体再骨折、下肢深静脉血栓、褥疮、肺炎等并发症发生率显著低于对照组,差异有统计学意义(P<0.05)。结论壮骨止痛胶囊对绝经后骨质疏松椎体压缩骨折PVP术后患者的应用能减少并发症的发生,能提高患者的骨密度,还能促进缓解胸腰背疼痛,改善患者的腰椎功能。
Objective To explore and analysis the effects of Zhuangguzhitong capsule on postmenopausal osteoporotic vertebral compression fractures after percutaneous vertebroplasty(PVP).Methods A total of 96 postmenopausal patients with osteoporotic vertebral compression fractures who completed PVP surgery in Qingyang Traditional Chinese Medicine Hospital from May 2019 to January 2022 were selected as the research subjects,and the patients were equally divided into the traditional Chinese medicine group and the control group accorded to the simple random number table method.The control group were given routine physical therapy,and the traditional Chinese medicine group were given Zhuanggu Zhitong Capsule on the basis of the treatment of the control group.The two groups were treated for 3 months.The VAS thoracolumbar back pain score,ODI spine score,lumbar vertebral bone mineral density changes,and complications during treatment were compared between the two groups before and after treatment.Results The VAS thoracolumbar back pain scores in two groups after treatment were lower than those before treatment(P<0.05),and the traditional Chinese medicine group were also signifi cantly lower than that in the control group(P<0.05).The ODI spine scores in two groups after treatment were signifi cantly lower than those before treatment(P<0.05),and the traditional Chinese medicine group were also signifi cantly lower than that in the control group(P<0.05).After treatment,the bone mineral density of lumbar vertebrae in two groups were significantly higher than that before treatment(P<0.05),and the traditional Chinese medicine group were also signifi cantly higher than that in the control group after treatment(P<0.05).The incidence of complications such as vertebral body refracture,lower extremity deep venous thrombosis,bedsore,and pneumonia in the traditional Chinese medicine group were signifi cantly lower than that in the control group(P<0.05).Conclusion The application of Zhuanggu Zhitong Capsules in postmenopausal osteoporotic vertebral compression fracture patients after PVP can reduce the incidence of complications,improve the bone mineral density of the patients,and also promote the relief of thoracolumbar back pain and improve the lumbar spine function of the patients.
