Klotho蛋白及血钙与重度子痫前期的关系研究

摘要 目的探讨血钙浓度与重度子痫前期(SPE)之间的关系及Klotho蛋白在重度子痫前期患者胎盘组织、母血及脐血中的表达和作用。方法收集2020年8月至2021年3月扬州大学临床医学院妇产科正常妊娠孕妇40例(对照组)和重度子痫前期孕妇43例(试验组),检测两者孕妇血钙浓度,通过免疫组化染色检测两组产妇胎盘组织中Klotho蛋白的蛋白表达,通过ELISA法检测母血、脐血中Klotho蛋白的浓度,并进行相关性分析,比较两组产妇的一般情况和产妇并发症及新生儿的结局情况。结果试验组孕妇血Ca^(2+)低于对照组,分别为(2.10±0.35)mmol/L、(2.20±0.31)mmol/L,差异有统计学意义(t=2.68,P<0.05);Klotho蛋白表达于胎盘合体滋养细胞、细胞滋养细胞及绒毛结缔组织间隙,试验组胎盘中Klotho蛋白表达低于对照组;母血中Klotho蛋白浓度低于对照组,分别为(148.97±68.80)pg/mL、(233.80±117.20)pg/mL,但差异无统计学意义(t=1.76,P>0.05),脐血中Klotho蛋白浓度低于对照组,分别为(62.73±28.86)pg/mL、(121.47±22.85)pg/mL,差异有统计学意义(t=4.51,P<0.05),试验组孕妇发生不良妊娠结局率亦高于对照组。在Pearson及Spearman相关性分析中发现,母血Klotho蛋白浓度与血压、剖宫产率、ICP呈负相关,脐血Klotho蛋白浓度与分娩孕周及胎龄呈正相关,与早产的发生呈负相关。结论SPE孕妇胎盘中Klotho蛋白的表达低于对照组,脐血中含量低于对照组,Klotho蛋白与SPE的发生有一定的关系。血Ca^(2+)、脐血中Klotho蛋白浓度的降低是发生SPE的高危因素,血Ca^(2+)、母血及脐血中Klotho蛋白浓度与妊娠结局及围产儿结局有一定的相关性,对SPE的预测、诊断有一定的意义。 Objective To investigate the relationship between blood calcium concentration and severe pre-eclampsia(SPE)and the expression and effect of Klotho protein in placental tissue,maternal blood,and umbilical cord blood of patients with SPE.Methods The 40 pregnant women with normal pregnancies(control group)and 43 pregnant women with SPE(test group)were collected from the Department of Obstetrics and Gynecology,Yangzhou University Medical College from August 2020 to March 2021,and the blood calcium concentration of both groups was measured.The expression of Klotho protein in the placental tissues of the two groups was detected by immunohistochemical staining,and the concentration of Klotho protein in maternal blood and umbilical cord blood was detected by ELISA,and correlation analysis was performed to compare the maternal general condition and maternal complications and neonatal outcomes of the two groups.Results The blood Ca^(2+) of pregnant women in the test group was lower than that of the control group,at(2.10±0.35)mmol/L and(2.20±0.31)mmol/L,respectively,with statistically significant differences(t=2.68,P<0.05).Klotho protein was expressed in placental syncytial trophoblasts,cytotrophoblasts,and interstitial chorionic connective tissue.Klotho protein expression in the placenta of the test group was lower than that of the control group.The concentration of Klotho protein in maternal blood was lower than that in the control group,at(148.97±68.80)pg/mL and(233.80±117.20)pg/mL respectively,but the difference was not statistically significant(t=1.76,P>0.05).The concentration of Klotho protein in cord blood was lower than that in the control group,at(62.73±28.86)pg/mL,and(121.47±22.85)pg/mL,respectively,with statistically significant differences(t=4.51,P<0.05).The incidence of adverse pregnancy outcomes was also higher in the test group than in the control group.In the Pearson and Spearman correlation analysis,maternal blood Klotho protein concentration was found to be negatively correlated with blood pressure,cesarean section rate,and ICP,and cord blood Klotho protein concentration was positively associated with the week of gestation and gestational age at delivery and negatively associated with the occurrence of preterm delivery.Conclusion The expression of Klotho protein in the placenta of SPE pregnant women was lower than that of the control group,the content in cord blood was lower than that of the control group,and there was a relationship between Klotho protein and the occurrence of SPE.The decrease in blood Ca^(2+) and cord blood Klotho protein concentration is a high-risk factor for the occurrence of SPE.There is a correlation between blood Ca^(2+),maternal blood and cord blood Klotho protein concentration and pregnancy outcome,and perinatal outcome,which has some significance for the prediction and diagnosis of SPE.
