摘要
目的探讨T淋巴细胞及相关因子水平在不同程度幽门螺杆菌(Hp)感染的胃溃疡患者体内水平差异。方法选取2018年10月-2020年10月首次在诸暨市人民医院就诊的127例Hp感染胃溃疡患者为研究组,选取同期30名入院行健康检查结果正常者为对照组;比较并分析入组患者病损面积、炎症因子水平、T淋巴细胞水平。结果根据研究对象黏膜状态、腺体形态、腺体密度的评分总和,对患者胃黏膜损害程度进行程度划分,分别为轻度损伤、中度损伤、重度损伤,不同损伤程度的患者胃黏膜各项评分差异有统计学意义(P<0.05);白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)水平随胃黏膜损害程度加重升高,转化生长因子-β(TGF-β)水平随着胃黏膜损害程度加重而下降,正常水平与不同损伤程度的患者血清IL-6、TNF-α、IL-10、TGF-β水平差异有统计学意义(P<0.05)。正常水平患者与不同损伤程度的患者T细胞亚群水平差异有统计学意义,重度损伤患者CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+)水平与其他程度比较差异有统计学意义(P<0.05)。结论IL-6、TNF-α、IL-10、TGF-β细胞因子水平和T淋巴细胞水平在幽门螺杆菌感染性胃溃疡发病程度中有重要作用。
OBJECTIVE To explore the differences in the levels of T lymphocytes and related factors in patients with different degrees of Helicobacter pylori(Hp)-infected gastric ulcer.METHODS A total of 127 patients aged 25-70 years with gastric ulcer caused by Hp infection who first visited Zhuji People′s Hospital from Oct 2018 to Oct 2020 were enrolled.30 healthy patients under physical examination during the same period were recruited as the normal group.The lesion area,inflammatory factor levels,and T lymphocyte levels between the two groups were compared.RESULTS According to the sum scores of the mucosal state,gland shape and gland density in the research subjects,the degrees of gastric mucosal damage of the patients were divided into the mild injury,moderate injury,and severe injury.The differences in the scores of gastric mucosa in patients with different degrees were significant(P<0.05).The levels of IL-6,TNF-α,and IL-10 increased significantly with the aggravation of gastric mucosal damage,while the level of TGF-βdecreased.There were significant differences in serum IL-6,TNF-α,IL-10 and TGF-βlevels between normal patients and patients with different degrees of injury(P<0.05).There were significant differences in the levels of T cell subsets between the normal group and patients with different degrees of injury.The differences in the levels of CD_(3)^(+),CD_(4)^(+),and CD_(8)^(+) between the severe injury group and other groups were significant(P<0.05).CONCLUSION The levels of IL-6,TNF-α,IL-10,TGF-βcytokines and T lymphocytes play an important role in the pathogenesis of Hp-infected gastric ulcer.
