摘要
目的:分析不同亚型高危型人乳头瘤病毒(high-risk human papilloma virus, HR-HPV)的感染情况与不同级别宫颈病变中的相关性,探讨HR-HPV在宫颈癌前病变及宫颈癌发生发展中的作用。方法:回顾性收集2016年1月至2020年12月在海南省海口市妇幼保健院行阴道镜检查及镜下活检的591例宫颈病变患者作为研究对象,根据病理检查结果按照宫颈病变严重程度将患者分为慢性宫颈炎(chronic cervicitis, CC)组(195例)、低级别鳞状上皮内病变(low-grade squamous intraepithelial lesion, LSIL)组(236例)、高级别鳞状上皮内病变(high-grade squamous intraepithelial lesion, HSIL)组(150例)、鳞状细胞癌(squamous of cervical carcinoma, SCC)组(10例),回顾性分析各组HR-HPV感染情况,对不同HR-HPV感染亚型与宫颈病变程度进行相关性分析。结果:CC、LSIL、HSIL和SCC组患者的HR-HPV感染率分别为28.2%(55/195)、79.7%(188/236)、93.3%(140/150)、100.0%(10/10),随着宫颈病变严重程度的增加,HR-HPV感染率也随之增加,差异有统计学意义(χ^(2)_(趋势)=172.464,P<0.001)。不同级别宫颈病变组,随着宫颈病变严重程度的增加,HR-HPV单一感染率也随之增加,差异有统计学意义(P<0.05),HR-HPV多重感染率之间随病变严重程度的增加亦呈升高趋势,但在SCC组的感染率稍有回落(P<0.05)。在不同级别宫颈病变患者及全部宫颈病变患者中,感染率最高的亚型均为HPV16,且HPV16的感染率随着病变严重程度的增加而升高,差异具有统计学意义(χ^(2)_(趋势)=14.805,P<0.001)。结论:HR-HPV感染率与宫颈病变的严重程度呈正相关关系,不同宫颈病变严重程度组患者HR-HPV的单一感染均明显大于多重感染率,本地区导致宫颈癌及癌前病变感染率最高的5种亚型依次为HPV16、52、58、18、51。
Objective: To analyze the correlation between different high-risk human papilloma virus(HR-HPV) subtypes and different levels of cervical lesions, and explore the significance of HR-HPV in cervical precancerous lesions and the occurrence and development of cervical cancer. Methods: A total of 591 patients who underwent colposcopy and biopsy in Haikou Hospital of Maternal and Child Health in Hainan Province from January 2016 to December 2020 were retrospectively collected. The patients were assigned to the CC group(chronic cervicitis, n=195), the LSD group(low-grade squamous intraepithelial lesion, n=236), the HSIL group(high-grade squamous intraepithelial lesion, n=150) and the SCC group(squamous cell carcinoma of cervix, n=10) according to pathological examination results and the severity of cervical lesions. The HR-HPV infection in each group was analyzed retrospectively, and the correlation between HR-HPV subtypes and the degree of cervical lesions was analyzed. Results: HR-HPV infection rates in CC, LSIL, HSIL and SCC groups were 28.2%(55/195), 79.7%(188/236), 93.3%(140/50) and 100.0%(10/10), respectively. With the increase of the severity of cervical lesions, the HR-HPV infection rate increased significantly(χ^(2)_(trend)=172.464,P<0.001);the trend in single infection remained in groups(P<0.05);and the multiple infection rate increased, while that in the SCC group decreased slightly(P<0.05). HPV16 was the subtype with the highest infection rate, and its infection rate significantly increased as cervical lesions aggravated(χ^(2)_(trend)=14.805,P<0.001). Conclusion: The infection rate of HR-HPV is positively correlated to the severity of cervical lesions. The single infection rate exceeds the multiple infection rate in different levels of cervical lesions. In this study, the five subtypes with the top 5 infection rates of cervical cancer or cervical precancerous lesions are HPV16, 52, 58, 18 and 51.
