摘要
目的探讨集束化护理措施对ICU机械通气的糖尿病患者呼吸机相关性肺炎的干预效果。方法选取2019年10月—2020年10月该院收治的110例糖尿病ICU机械通气治疗的患者为研究对象,以护理模式的不同分为两组,每组55例。常规组选择常规护理,研究组选择集束化护理干预。通过自行研制的满意度调查问卷对患者的满意度进行评估,分析并发症发生情况,包括呼吸机相关性肺炎及呼吸衰竭,机械通气时间及住院时间,血气指标通过SaO_(2)、PaO_(2)、PaCO_(2)进行评估。结果研究组护理满意度为98.18%(54/55)高于常规组的满意度83.64%(46/55),差异有统计学意义(χ^(2)=7.040,P<0.05)。研究组机械通气时间及住院时间分别为(5.42±1.23)、(8.40±1.13)d短于常规组的(7.24±1.10)、(12.86±2.61)d,差异有统计学意义(P<0.05)。研究组患者的SaO_(2)、PaO_(2)、PaCO_(2)指标分别为(97.13±1.22)%、(90.10±1.33)mmHg、(31.53±1.33)mmHg,均优于常规组,差异有统计学意义(P<0.05)。研究组患者并发症总发生率较常规组更低,差异有统计学意义(P<0.05)。结论对糖尿病ICU进行机械通气治疗患者实施集束化护理干预后,能够有效降低呼吸机相关性肺炎的发生,效果显著。
Objective To investigate the intervention effect of cluster nursing measures on ventilator-associated pneumonia in patients with diabetes mellitus mechanically ventilated in ICU.Methods Selected 110 patients with diabetes ICU treated by mechanical ventilation in the hospital from October 2019 to October 2020 as the research objects,divided into two groups according to the different nursing modes,55 cases in each group,the conventional group chose routine nursing,and the study group selected the cluster nursing intervention.Through the self-developed satisfaction questionnaire,the satisfaction of patients was evaluated,and the occurrence of complications,include ventilator-associated pneumonia,respiratory failure were analyzed,mechanical ventilation time,hospital stay and blood gas indexes by SaO_(2),PaO_(2) and PaCO_(2) were evaluated.Results The nursing satisfaction of the study group was 98.18%(54/55),higher than 83.64%(46/55)in the conventional group,and the difference was statistically significant(χ^(2)=7.040,P<0.05).The mechanical ventilation time and hospitalization time of the study group was(5.42±1.23)d and(8.40±1.13)d,shorter than(7.24±1.10)d and(12.86±2.61)d in the conventional group,and the differencewas statistically significant(P<0.05).The indexes of SaO_(2),PaO_(2) and PaCO_(2) in the study group were(97.13±1.22)%,(90.10±1.33)mmHg and(31.53±1.33)mmHg,respectively,which were better than those in the conventional group,and the difference was statistically significant(P<0.05).The total incidence of complications in the study group was lower than that in the conventional group,and the difference was statistically significant(P<0.05).Conclusion After the cluster nursing intervention for the patients with mechanical ventilation in the diabetic ICU,the incidence of ventilator-associated pneumonia can be effectively reduced,and the effect is significant.
