摘要
目的检索、评价与整合术中压力性损伤预防的最佳证据,旨在为临床实践提供科学、可操作性的实践性指导。方法计算机检索医脉通指南网、美国指南网、苏格兰校际指南网、英国国家卫生医疗质量标准署、新西兰指南工作组、安大略注册护士协会、PubMed、万方、CNKI、维普、Cochrane Library、JBI Library、CINAHL、EMBASE等数据库中与术中压力性损伤预防相关的临床实践指南、系统评价或Meta分析、RCT。由2名研究者对纳入的文献质量进行评价,并对符合质量标准的文献进行证据提取与汇总。结果共纳入16篇文献,其中指南8篇,系统评价或Meta分析6篇,RCT 2篇。最终形成24条术中压力性损伤预防方案,包括7个方面:风险因素及风险评估、预防性皮肤护理、体位管理、支撑面、器械相关压力性损伤预防、体温管理、交接与转运,为临床压力性损伤预防提供规范和指引。结论总结的目前关于术中压力性损伤预防的最佳证据可为医护人员提供循证依据。
Objective To search,evaluate and put together the best evidence for the prevention of intraoperative pressure injury and therefore to provide scientific and operational guidance for clinical practice.Methods Literature of guidelines,systematic review or meta-analysis and randomised controlled trials on prevention of intraoperative pressure injury was searched through databases such as Medical Masscom Guide Network,National Guideline Clearinghouse,Scottish Intercollegiate Guidelines Network,National Institute for Health and Clinical Excellence,New Zealand Guidelines Group,Registered Nurses Association of Ontario,PubMed,Wanfang database,CNKI,the Cochrane Library,JBI Library,CINAHL and EMBASE.The included literature was evaluated by two researchers,who carried out evidence extraction and summarization from the literature meeting the quality standards.Results In total,8 guidelines for clinical practice,6 systematic reviews or meta-analyses and 2 RCT articles were included.A plan that included 24 items for prevention of intraoperative pressure injury was worked out with seven listed dimensions:risk factors and risk assessment,preventive skin care,position management,supporting surface,prevention of device-related pressure injury,temperature management and the handover and transport.Conclusions The review of the best available evidence on the prevention of intraoperative pressure injury provides evidence-based references for health care professionals.
