摘要
目的探讨新生儿脐血雄性激素的影响因素,为促进孕妇孕期及新生儿保健提供指导。方法选取2018年4月至2019年1月在蚌埠市第三人民医院分娩的孕妇和新生儿各342例为研究对象,调查孕妇一般人口学特征,孕期饮食、运动及营养补充剂摄入等因素;检测新生儿脐血睾酮(T)、游离睾酮(FT)、双氢睾酮(DHT)、脱氢表雄酮(DHEA)和性激素结合球蛋白(SHBG)水平。结果孕期服用锌制剂增加男性新生儿脐血对数转换后脱氢表雄酮(LnDHEA)水平[SE=0.19(0.22)],顺产增加男、女新生儿脐血对数转换后睾酮(LnT)水平[SE=0.25(0.05),SE=0.31(0.07)],非初产降低男性新生儿脐血对数转换后性激素结合球蛋白(LnSHBG)水平[SE=-0.10(0.06)]。孕妇分娩年龄、孕期不健康饮食与女性新生儿脐血LnDHEA呈正相关[SE=0.21(0.01),SE=0.21(0.13)],吸烟降低女性新生儿脐血LnT水平[SE=-0.17(0.03)],孕妇身高与女性新生儿脐血LnSHBG呈正相关[SE=0.28(0.01)]。结论孕妇分娩年龄、孕期服用锌制剂、分娩方式、不健康饮食、吸烟、身高等可能与新生儿脐血雄性激素水平有关。
Objective To explore the influencing factors of umbilical cord blood androgen level in neonates,so as to provide guidance for promoting pregnancy and neonatal health care.Methods Three hundred and forty-two pregnant women and their neonates delivered in the Third People’s Hospital of Bengbu from April 2018 to January 2019 were selected as the research objects.General demographic characteristics of pregnant women,and their diet,exercise and nutrition supplement intake during pregnancy were investigated.Levels of testosterone(T),free testosterone(FT),dihydrotestosterone(DHT),dehydroepiandrosterone(DHEA)and sex hormone binding globulin(SHBG)in neonatal cord blood were detected.Results Zinc supplementation during pregnancy increased the level of LnDHEA(DHEA after logarithmic conversion)in the cord blood of male neonatesSE=0.19(0.22).Compared with cesarean section,natural labor increased the level of LnT(T after logarithmic conversion)in the cord blood of male and female neonatesSE=0.25(0.05),SE=0.31(0.07).Non-first delivery reduced the level of LnSHBG(SHBG after logarithmic conversion)in the cord blood of male and female neonates SE=-0.10(0.06).Maternal age and unhealthy diet during pregnancy were positively correlated with the LnDHEA level in the cord blood of female neonates SE=0.21(0.01),SE=0.21(0.13).Smoking decreased LnT level in the cord blood of female neonates SE=-0.17(0.03).Maternal height was positively correlated with the LnSHBG level in the cord blood of female neonatesSE=0.28(0.01).Conclusion Maternal age,height,Zinc supplementation during pregnancy,unhealthy diet during pregnancy,smoking during pregnancy,and mode of deliverymay be related to neonatal cord blood androgen level.
