摘要
目的了解我国孕早期女性抑郁现状,并探究孕前及孕早期烟草暴露与孕早期抑郁的关系,为孕早期抑郁的预防提供理论依据。方法本研究相关数据来源于中国孕产妇队列研究(CPWCS)协和项目,即2017年7月25日至2018年7月24日在全国15个省份的24家医院收集的7 976名孕早期孕妇的数据。采用线上调查方式调查一般资料和孕前及孕早期烟草暴露情况,采用爱丁堡产后抑郁量表评估孕早期妇女抑郁情况。采用SPSS 20.0软件进行χ^2检验和多因素非条件logistic回归。结果本研究孕早期孕妇抑郁的发生率为46.82%。3.01%的孕妇在孕前有主动吸烟行为,1.09%的孕妇孕早期有主动吸烟行为,43.34%的孕妇孕早期存在被动吸烟。孕前主动吸烟孕妇的抑郁发生率(56.67%)高于不吸烟孕妇(46.51%),孕早期被动吸烟孕妇的抑郁发生率(50.27%)高于不被动吸烟孕妇(44.17%),差异均有统计学意义(P<0.01);孕早期主动吸烟孕妇的抑郁发生率(55.17%)和不主动吸烟孕妇(46.72%)的差异无统计学意义(P>0.05)。多因素非条件logistic回归分析结果显示,孕前主动吸烟(OR=1.386,95%CI:1.043~1.841)和孕早期被动吸烟(OR=1.320,95%CI:1.206~1.445)与孕早期抑郁均有显著关联,均有统计学意义(P<0.05,P<0.01);未发现孕早期主动吸烟与孕早期抑郁的关联性,无统计学意义(P>0.05)。结论孕前主动吸烟及孕早期被动吸烟与孕早期女性抑郁有关,应采取措施降低孕妇烟草暴露。
Objective To understand the prevalence of first trimester depression in China and the association of tobacco exposure before pregnancy and early pregnancy with first trimester depression,and to provide the theory basis for prevention of first trimester depression.Methods Data was from Concorde project of the Chinese Pregnant Women Cohort study(CPWCS),i.e.the data of7 976 first trimester women in 24 hospitals among 15 provinces from July 25 th of 2017 to July 24 th of 2018.Self-designed online questionnaire was used to collect the general information and tobacco exposure before pregnancy and early pregnancy.Edinburgh postnatal depression scale(EPDS)was used to assess the depression in the first trimester women.Theχ^2 test and multivariable logistic regression were used to analyze the data,the used software was SPSS 20.0.Results The incidence rate of depression in first trimester women was 46.82%.The 3.01%of pregnancy women had the active smoking before pregnancy,1.09%of pregnancy women had the active smoking in early pregnancy,43.34%of pregnancy women had the passive smoking in early pregnancy.The depression rate(56.67%)in pregnancy women with active smoking before pregnancy was significantly higher that(46.51%)in non-smoking pregnancy women;the depression rate(50.27%)in pregnancy women with passive smoking in early pregnancy was significantly higher that(44.17%)in pregnancy women without passive smoking in early pregnancy(P<0.01);the depression rate(55.17%)in pregnancy women with active smoking in early pregnancy was not significantly higher that(46.72%)in pregnancy women without active smoking in early pregnancy(P>0.05).Multivariable logistic regression results demonstrated that prenatal active smoking(OR=1.386,95%CI:1.043-1.841)and passive smoking in early pregnancy(OR=1.320,95%CI:1.206-1.445)were statistically associated with depression in first trimester(P<0.05 or P<0.01).The association of active smoking during the early pregnancy with depression was not found(P>0.05).Conclusion Prenatal active smoking and passive smoking during early pregnancy were associated with depression in first trimester.Some intervention measures should be taken to reduce the tobacco exposure in pregnancy women.
