摘要
目的探究刃针搜法治疗脑梗死后感觉障碍的临床作用机制。方法脑梗死后感觉障碍患者90例随机分为刃针治疗组、针刺对照组、药物对照组各30例。分别采用刃针搜法治疗、常规针刺治疗、甲钴胺注射治疗。同时进行治疗前后的TIMP-1等相关指标检测并进行Fugl-Meyer评分比较。同时进行体感诱发电位(SEP)N20等相关指标检测并比较。结果(1)治疗后刃针治疗组的Fugl-Meyer评分与药物对照组和针刺对照组治疗后评分相比较的结果显著提高且优于两对照组,差异具有统计学意义(P<0.05)。(2)治疗后刃针治疗组金属蛋白酶-1(TIMP-1)指标显著上升且优于两对照组,差异具有统计学意义(P<0.05)。(3)治疗后刃针治疗组的患侧体感诱发电位(SEP)N20较药物对照组和针刺对照组潜伏期明显缩短、波幅明显增高且优于两对照组,差异具有统计学意义(P<0.05)。结论本课题研究证明刃针搜法可以通过调节TIMP-1进而改善神经元功能,并促进感觉神经传导通路的修复。
Objective To explore the clinical mechanism of blade needle search method in the treatment of sensory disturbance after cerebral infarction.Methods 90 patients with sensory disturbance after cerebral infarction were randomly divided into knife needle treatment group,acupuncture control group and drug control group with 30 cases in each group.They were treated with blade needle searching,conventional acupuncture and mecobalamin injection.At the same time,TIMP-1 and other related indexes were detected before and after treatment,and Fugl Meyer score was compared.At the same time,somatosensory evoked potential(SEP)N20 and other related indicators were detected and compared.Results(1)After treatment,the Fugl Meyer score of the blade acupuncture treatment group was significantly higher than that of the drug control group and the acupuncture control group,and was significantly better than that of the two control groups(P<0.05).(2)after treatment,the index of metalloproteinase-1(TIMP-1)in the blade needle treatment group increased significantly and was better than that in the two control groups,with statistical significance(P<0.05).(3)After treatment,the latency and amplitude of SEP N20 in the treatment group were significantly shorter than those in the drug control group and the acupuncture control group,and the amplitude was significantly higher than those in the two control groups,with statistical significance(P<0.05).Conclusion The therapeutic effect can be achieved by improving the blood circulation and blood supply of brain tissue,improving the function of neurons and promoting the repair of sensory nerve conduction pathway.
