摘要
目的探讨龙虎交战针法联合腰背部肌肉拉伸练习治疗腰椎间盘突出症(LDH)的临床效果。方法选取2019年12月—2020年12月收治的LDH 116例,根据针灸方法不同分为研究组与对照组,每组58例。两组均采用腰背部肌肉拉伸练习,在此基础上研究组采用龙虎交战针法,对照组采用平补平泻针法,均治疗20 d。观察两组临床疗效,比较治疗前后直腿抬高角度(ASLR)、血清5-羟色胺(5-HT)和一氧化氮(NO)水平,以及日本骨科协会(JOA)、Oswestry功能障碍指数(ODI)和疼痛视觉模拟评分法(VAS)评分。结果研究组总有效率高于对照组(P<0.01)。治疗后,两组ASLR较治疗前增大,且研究组大于对照组(P<0.01)。治疗后,两组血清5-HT及NO水平较治疗前降低,且研究组低于对照组(P<0.01)。治疗后,两组ODI、VAS评分较治疗前降低,JOA评分较治疗前增高;且研究组ODI、VAS评分低于对照组,JOA评分高于对照组(P<0.01)。结论龙虎交战针法联合腰背部肌肉拉伸练习可有效减轻LDH患者疼痛程度,增大ASLR,改善腰椎功能,并减少血清炎性因子含量,利于提高疗效。
Objective To explore the clinical effectiveness of dragon-tiger contending needling combined with lumbar back muscle stretching exercises in treatment of patients with lumbar intervertebral disc herniation(LDH).Methods A total of 116 patients with LDH admitted between December 2019 and December 2020 were selected and divided into research group(n=58)and control group(n=58)according to different acupuncture methods.Both groups received lumbar back muscle stretching exercises.Research group was added with the dragon-tiger contending needling method,while control group was added with mild reinforcing-reducing needling method,and both groups were treated for 20 d.Clinical efficacies were observed,and levels of angle during active straight leg raising(ASLR),serum 5-hydroxytryptamine(5-HT)and nitric oxide(NO),and scores of the Japanese Orthopaedic Association(JOA),Oswestry dysfunction Index(ODI)and Pain Visual Analogue Scale(VAS)before and after treatment were compared in two groups.Results The total effective rate in research group was significantly higher than that in control group(P<0.01).After treatment,ASLR levels were significantly increased compared with those before treatment in two groups,and the level in research group was significantly higher than that in control group(P<0.01);levels of serum 5-HT and NO were significantly lower than those before treatment in two groups,and the levels in research group were significantly lower than those in control group(P<0.01).After treatment,ODI and VAS scores were significantly lower,while JOA scores were significantly higher than those before treatment in two groups;compared with those in control group,ODI and VAS scores were significantly lower,while the JOA score was significantly higher in research group(P<0.01).Conclusion Dragon-tiger contending needling combined with lumbar back muscle stretching exercises may effectively reduce pain,increase ASLR,improve lumbar function and reduce contents of serum inflammatory factors in treatment of patients with LDH,which is conductive to improvement in the efficacy.
