摘要
子宫瘢痕憩室的形成与剖宫产相关,其常见的临床表现为剖宫产后经期延长。邓高丕教授认为本病乃因金刃所伤而致瘀,瘀血稽留体内,阻滞胞宫、胞脉、胞络所致。治疗上遵循胞宫定期藏泻之性,在行经期应用通因通用法,治疗子宫瘢痕憩室引起的经期延长。当临床表现为瘀阻冲任,血行不畅致经期延长伴量不多而点滴难净者,重在行经期使用活血通经法,祛瘀血以生新血;当临床表现为瘀阻冲任,血不循经而致经期延长伴月经量多者,重在行经期使用化瘀止血法,止血而不留瘀;子宫瘢痕憩室乃因剖宫产所致,质本属瘀,瘀邪作祟表现为行经期的经期、经量问题者,当采用通因通用之法,配合瘀血成因,以助力血行,使之如趁风扬帆、顺水推舟,瘀有出路,则收事半功倍之效。
The formation of previous cesarean scar defect(PCSD)is related to the cesarean section.The common clinical manifestation is prolonged menstruation.Professor Deng Gaopi considers that the disease results from blood stasis caused by blade injury during operation,which blocks the uterus,uterine vessels,and uterine collaterals.He observes the regular nature of storing and reducing of the uterus,treats incontinent syndrome with dredging method when menstrual period,to treat PCSD induced prolonged menstruation.When the clinical manifestation is blood stasis blocking thoroughfare and conception channels stasis,and poor blood flow showing in the form of the prolonged menstrual period with continuous small amount leakage,the treatment should focus on activating blood to promote menstruation and remove blood stasis to produce new blood.When the manifestation is blood stasis blocking thoroughfare and conception channels,blood fails to circulate in the vessels,which leads to prolonged menstrual period and heavy menstrual flow,the treatment should emphasize removing blood stasis to stop bleeding without leaving stasis.PCSD is caused by cesarean section and belongs to blood stasis diseases.If the stasis manifested itself in the form of abnormal menstrual period or volume during menstruation,treating incontinent syndrome with dredging method should be adopted to cooperate with the stasis root cause to active the blood,using it as favorable wind and downstream water to a boat,finding the way out for blood stasis,and the result is twice the result with half the effort。
