分析不同机械通气方式治疗重症新生儿呼吸窘迫综合征近远期效果 被引量:2

Analysis of the Short-term and Long-term Effects of Different Mechanical Ventilation Methods in the Treatment of Severe Neonatal Respiratory Distress Syndrome

摘要 目的观察为重症新生儿呼吸窘迫综合征(NRDS)患儿实施不同机械通气方式治疗的近远期效果。方法2017年4月-2018年4月在该院接受重症新生儿呼吸窘迫综合征治疗的70例患儿,按照抽签法均分样本为高频振荡组和间歇指令组,间歇指令组选择间歇指令通气法,高频振荡组选择高频振荡通气法,对比两组临床疗效、并发症发生率、出院后1年复发率治疗前后的血气指标以及临床指标情况。结果高频振荡组治疗总有效率(97.14%)显著高于间歇指令组(71.43%),差异有统计学意义(χ^(2)=8.736,P=0.003);高频振荡组并发症总发生率(5.71%)明显低于间歇指令组(28.57%),差异有统计学意义(χ^(2)=6.436,P=0.011);治疗前,两组动脉血氧分压、动脉二氧化碳分压以及血氧饱和度等数据指标十分相近,差异无统计学意义(P>0.05);治疗后,高频振荡组动脉血氧分压、动脉二氧化碳分压以及血氧饱和度等数据指标均优于间歇指令组,差异有统计学意义(P<0.05);高频振荡组氧疗时间、通气时间以及住院时间均少于间歇指令组,差异有统计学意义(P<0.05);高频振荡组出院后1年的复发率(22.85%)低于间歇指令组,差异有统计学意义(P<0.05)。结论将不同机械通气方式应用于重症新生儿呼吸窘迫综合征患儿临床治疗中,高频振荡通气法的治疗效果更佳,产生的并发症较少,安全性较高,复发率更低可以明显改善血气指标,促进患儿康复,值得进一步研究应用。 Objective To observe the short-term and long-term effects of different mechanical ventilation treatments for children with severe neonatal respiratory distress syndrome(NRDS).Methods 70 children who were treated for severe neonatal respiratory distress syndrome in the hospital from April 2017 to April 2018.According to the lottery method,the samples were divided into high-frequency oscillation group and intermittent instruction group,intermittent instruction group chose the intermittent mandatory ventilation method,and the high-frequency oscillation group chose the high-frequency oscillation ventilation method.The clinical efficacy,incidence of complications,recurrence rate 1 year after discharge blood gas indexes before and after treatment and clinical indexes were compared between the two groups.Results The total effective rate of treatment in the high-frequency oscillation group(97.14%)was significantly higher than the intermittent instruction group(71.43%),and the difference was statistically significant(χ^(2)=8.736,P=0.003);the total incidence of complications in the high-frequency oscillation group(5.71%)was significantly lower than the intermittent command group(28.57%),and the difference was statistically significant(χ^(2)=6.436,P=0.011);before treatment,the two groups of arterial blood oxygen partial pressure,arterial carbon dioxide partial pressure,and blood oxygen saturation were very similar and the difference was not statistically significant(P>0.05).After treatment,the high-frequency oscillation group arterial blood oxygen partial pressure,arterial carbon dioxide partial pressure,and blood oxygen saturation were better than the intermittent instruction group,and the difference was statistically significant(P<0.05);the high-frequency oscillation group oxygen therapy time and ventilation time and hospital stay were shorter than those in the intermittent instruction group,and the difference was statistically significant(P<0.05);1 year after discharge,the recurrence rate of high-frequency oscillation group(22.85%)was lower than that of intermittent instruction group,the difference was statistically significant(P<0.05).Conclusion Applying different mechanical ventilation methods to the clinical treatment of children with severe neonatal respiratory distress syndrome,high-frequency oscillatory ventilation has better therapeutic effect,fewer complications,higher safety,lower recurrence rate and can significantly improve blood gas indicators,to promote the rehabilitation of children,it is worthy of further research and application.
