摘要
目的评价磁共振弥散张量成像(DTI)对创伤性脑损伤(traumatic brain injury,TBI)的诊断价值。方法选取25例创伤性脑损伤患者和25名正常成年人,分别进行DTI检查和常规磁共振扫描。行DTI图像后处理得到不同感兴趣区域(region of interest,ROI)各向异性分数(FA),包括两侧内囊前后支、胼胝体膝和压部的FA值。比较具有相同ROI的两组FA值,并将TBI组ROI的患侧和健康侧的FA值进行比较。对TBI组FA值与格拉斯哥转归评分(GOS)进行Pearson相关分析。结果TBI组患者内囊后支、额叶白质、顶叶白质FA值、ADC测定值均低于对照组,差异有统计学意义(P<0.05)。经Pearson相关分析,TBI组患者内囊后支FA值、顶叶白质FA值及额叶白质ADC值、顶叶白质ADC值与GOS评分显著相关(P<0.05)。ROC曲线分析显示,内囊后支FA值、顶叶白质FA值、额叶白质ADC值及顶叶白质ADC值对转归判断中的AUC分别为0.864(95%CI:0.688~1.000)、0.772(95%CI:0.571~0.973)、0.829(95%CI:0.653~1.000)、0.877(95%CI:0.735~1.000)。结论DTI对白质纤维的损伤更敏感,可为早期诊断TBI提供证据。
Objective To evaluate the diagnostic value of diffusion tensor imaging(DTI)for traumatic brain injury(TBI).Methods Twenty-five patients with TBI and 25 normal adults were selected and examined by conventional MRI scan and DTI.After DTI image processing,the fractional anisotropy(FA)of different regions of interest(ROI)were obtained,including FA values of bilateral internal capsule knees and hind limbs,corpus callosum knees and compression parts.The FA values of the two groups with the same ROI,and the FA values of the affected and healthy sides of the ROI in the TBI group were compared.Pearson correlation analysis was performed between the FA value and the Glasgow outcome score(GOS)of the TBI group.Results Compared with the control group,patients in the TBI group had lower FA and ADC values in the posterior branch of the internal capsule,frontal white matter and temporal white matter(P<0.05).Pearson correlation analysis showed that FA value of posterior branch of internal capsule,FA value of parietal white matter,ADC value of frontal white matter and ADC value of parietal white matter were correlated with GOS score in TBI group(P<0.05).ROC curve analysis showed that the AUC of the posterior branch of the internal capsule,the white matter of the parietal lobe,the white matter ADC of the frontal lobe and the white matter of the parietal lobe were 0.864(95%CI:0.688-1.000),0.772(95%CI:0.571-0.973),0.829(95%CI:0.653-1.000),0.877(95%CI:0.735-1.000).Conclusions DTI is more sensitive to the damage of white matter fibers,which can provide evidence for the early diagnosis of TBI.
