Application of MR Diffusion Tensor Imaging on Diagnosis of Mild Traumatic Brain Injury

摘要 目的探讨MR扩散张量成像(DTI)对轻微创伤性脑损伤(mTBI)的诊断价值。方法应用DTI对33例mTBI患者(外伤后2~14天,常规CT和MRI均为阴性)和33名健康志愿者(对照组)进行检查,在各向异性(FA)图上分别测量mTBI组和对照组两侧额叶、半卵圆中心、胼胝体膝部、压部、内囊前后肢及顶枕叶白质区的FA值,并进行两两比较。结果与对照组相比,mTBI组各区域的FA值均有不同程度的下降,差异有统计学意义(P<0.01)。结论 DTI能较为敏感地检测出mTBI的白质纤维的损伤情况,有助于早期诊断和改善预后。 Objective To explore the value of MR diffusion tensor imaging(DTI)on diagnosis of mild traumatic brain injury(mBTI).Methods 33 patients with mTBI(2-14 days after injury) whose routine CT and MRI were normal and matched 33 control subjects were performed with DTI.Fractional anisotropy(FA) was calculated from different white matter regions including frontal lobe,semioval center,corpus callosum,internal capsule and parieto-occipital and was also compared between mTBI and control group.Results Compared with control group,FA value was decreased in different degree in different white matter regions(P〈0.01).Conclusion DTI is sensitive for detecting the traumatic injury of white matter,which is helpful for diagnosis earlier and improving prognosis
