低剂量雌激素对围绝经期综合征女性患者性激素水平、骨代谢、骨密度、子宫内膜厚度的影响 被引量:7

Effects of low-dose estrogen on sex hormone levels,bone metabolism,bone mineral density and endometrial thickness in women with perimenopausal syndrome

摘要 目的探讨短期应用小剂量雌激素对更年期女性患者性激素、骨代谢指标、骨密度、子宫内膜厚度的影响。方法选取92例围绝经期综合征女性患者,随机分为观察组和对照组,每组46例。对照组予以口服醋酸甲羟孕酮治疗,观察组在对照组治疗的基础上加用口服戊酸雌二醇片。治疗6个月后观察2组患者Kupperman评分及临床结局,评估低剂量雌激素治疗前后2组患者体内性激素水平(LH、E_(2)、FSH)、骨代谢指标(TP1NP、β-CTX、ALP)水平及骨密度、子宫内膜厚度的变化情况。结果观察组患者经过治疗后的整体有效率为94.01%,对照组为71.08%,差异有统计学意义(P<0.05);2组患者Kupperman评分较治疗前均显著下降,观察组下降更为明显(P<0.05);治疗后观察组E_(2)、ALP、P1NP水平高于对照组(P<0.05),LH、FSH、CTX水平低于对照组(P<0.05),观察组骨密度水平高于对照组(P<0.05)。治疗前后观察组子宫内膜厚度与对照组比较差异无统计学意义(P>0.05)。结论联用小剂量醋酸甲羟孕酮及戊酸雌二醇片对治疗围绝经期综合征女性有明显效果,可改善围绝经期女性自主神经功能错乱症状,在改善骨代谢相关指标并在此基础上能大大提高患者的骨密度,对子宫内膜厚度几乎无影响。 Objective To investigate the effects of short-term low-dose estrogen on sex hormones,bone metabolism,bone density and endometrial thickness in menopausal female patients.Methods 92 female patients with perimenopausal syndrome were selected and randomly divided into the study group and the control group,46 cases in each group.The control group was treated with oral medroxyprogesterone acetate and the observation group were treated with oral pentanoic acid estradiol tablets on the basis of the control group,After 6 months treatment,the Kupperman scores and clinical outcome of patients in both groups were observed,and the levels of gonadal hormone(LH,E2,FSH)in the body,bone metabolic indexes(TP1NP,β-CTX,ALP)and the change of bone mineral density,endometrial thickness were measured.Results After treatment,the overall effective rate of the study group was 94.01%,and that of the control group was 71.08%,the difference was statistically significant(P<0.05).The Kupperman score in both groups was significantly lower than that before treatment,and the decrease was more obvious in the observation group(P<0.05).After treatment,the levels of E2,ALP and P1NP in the observation group were higher than those in the control group(P<0.05),while the levels of LH,FSH and CTX were lower than those in the control group(P<0.05).The BMD of the observation group was higher than that in the control group(P<0.05).There was no significant difference in endometrial thickness between the two groups(P>0.05).Conclusion The combination of low-dose of medroxyprogesterone acetate and pentanoic acid estradiol tablets in the treatment of perimenopausal syndrome women have obvious effect,which can improve perimenopausal women autonomic nervous function disorder symptoms,to improve the bone metabolism related indicators and can greatly improve the patient's bone mineral density,the endometrial thickness almost no impact.
