损伤控制复苏对创伤性休克患者疗效及血流动力学的影响 被引量:3

摘要 目的:探讨损伤控制复苏对创伤性休克患者的疗效及对血流动力学的影响。方法:选择2019年2月至2020年2月我院收治103例创伤性休克患者,采用随机数表法分为损伤控制组(n=52)和积极复苏组(n=51)。积极复苏组给予积极液体复苏治疗,损伤控制组采用损伤控制复苏治疗。比较两组临床疗效、平均动脉压(MAP)、血氧饱和度(SpO 2)、心率(HR)、凝血功能、格拉斯哥昏迷量表(GCS)、休克指数(SI)及并发症、存活率情况。结果:治疗后,损伤控组总有效率86.54%,较积极复苏组64.71%更高(P<0.05);两组MAP、SpO 2均随着时间的推移而升高,且损伤控制组高于积极复苏组(P<0.05);HR均随着时间的推移而降低,且损伤控制组低于积极复苏组(P<0.05);两组患者凝血功能均均随着时间的推移而降低,且损伤控制组低于积极复苏组(P<0.05);两组患者GCS随着时间的推移而升高,且损伤控制组高于积极复苏组(P<0.05);SI均随着时间的推移而降低,且损伤控制组低于积极复苏组(P<0.05);损伤控制组不良反应总发生率低于积极复苏组,且存活率高于积极复苏组(P<0.05)。结论:在创伤性休克中应用损伤控制复苏效果显著,可有效改善患者血流动力学,减少不良反应。 Objective:To study curative efficacy of injury control resuscitationin treatment of traumatic shock and its effects on Hemodynamics.Methods:103 patients of who received therapy from February 2019-February 2020 were selected as research objects.The control group was treated with active liquid recovery,while the observation group was treated with damage control resuscitation.Then clinical efficacy,mean arterial pressure(map),blood oxygen saturation(SpO2),heart rate(HR),blood coagulation function,Glasgow Coma Scale(GCS),shock index(SI),complications and survival rate were compared.Results:After treatment,the total effective rate of the injury control group was 86.54%,higher than that of the single drug group(64.71%),with a significant difference(P<0.05).After treatment,MAP and SpO2 of both groups increased over time,and HR of the injury control group was higher than that of the active resuscitation group,while HR of the injury control group was lower than that of the active resuscitation group(P<0.05).After treatment,the coagulation function of patients in both groups decreased over time,and the injury control group was lower than that of the active resuscitation group(P<0.05).After treatment,GCS of patients in both groups increased over time,and SI in the injury control group was higher than that in the active resuscitation group,while SI in both groups decreased over time,and the combined group was lower than that in the single drug group,and the injury control group was lower than that in the active resuscitation group(P<0.05).The incidence of adverse reactions in the injury control group was lower than that in the active resuscitation group,and the survival rate was higher than that in the active resuscitation group(P<0.05).Conclusion:The application of injury control resuscitation in traumatic shock has a significant effect,which can effectively improve the hemodynamics of patients and reduce adverse reactions.
