摘要
目的探讨专科护士主导的群组管理模式在维持性血液透析患者治疗依从性中的应用效果。方法选择2018年10月—2019年4月在本院透析中心行维持性血液透析的患者作为研究对象,按照患者透析时间和班次不同分为观察组(41例)和对照组(42例)两组。观察组进行为期4周的群组管理模式,对照组采用常规管理。观察4周后患者的治疗依从性和健康素养和干预后6个月内患者液体摄入依从性。结果干预结束时,观察组患者的健康素养得分明显高于对照组,差异具有统计学意义(P<0.05);观察组患者的饮食、液体摄入得分明显高于对照组,差异具有统计学意义(P<0.05);而两组患者的用药、透析方案及总得分比较差异不具有统计学意义(P>0.05);干预结束后6个月内,观察组患者平均透析间期体质量增加相对值低于对照组,液体摄入依从性好于对照组。结论专科护士主导的以控制液体摄入为主题群组管理模式可有效提高患者疾病相关健康素养、促进治疗依从性,降低透析间期体质量增加相对值,具有一定的实践意义。
Objective To explore the application effect of group management mode led by specialized nurses in treatment compliance of maintenance hemodialysis patients.Methods A total of 83 patients receiving maintenance hemodialysis in the hospital from October 2018 to April 2019 were enrolled as study subjects,and they were divided into the experimental group(41)and the control group(42)according to the different dialysis time and shift.The experimental group was treated with group management mode for 4 weeks,while the control group was treated with routine management.Patients'treatment compliance and health literacy at 4 weeks,and fluid intake compliance at 6 months after intervention were observed.Results At the end of intervention,the score of health literacy in the experimental group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The scores of diet and fluid intake in experimental group were significantly higher than those in control group,and the differences were statistically significant(P<0.05).There was no significant difference in medication,dialysis regimen and total scores between the two groups(P>0.05).Within 6 months after the end of intervention,the experimental group had a lower average body weight increase during dialysis than the control group,and the fluid intake compliance was better than the control group.Conclusions The group management mode led by specialized nurses could effectively improve disease-related health literacy of patients,promote treatment compliance,and reduce the relative value of body weight increase in the inter-dialysis period,which has certain practical significance.
