摘要
目的观察独活寄生汤联合中药艾灸治疗类风湿关节炎(RA)临床疗效及其对基质蛋白酶-3(MMP-3)、核因子-?B受体激活剂配体(RANKL)、骨保护素(OPG)的影响。方法将我院68例RA患者采用随机数字表法分为观察组与对照组各34例。对照组患者口服塞来昔布胶囊和甲氨喋呤,观察组患者在对照组治疗基础上加用独活寄生汤联合中药艾灸治疗,比较两组患者治疗总有效率、类风湿因子(RF)、红细胞沉降率(ESR)、C反应蛋白(CRP)、MMP-3、RANKL和OPG水平。结果观察组患者治疗总有效率高于对照组(P<0.05);治疗后,两组患者类风湿因子、红细胞沉降率、C反应蛋白均下降(P<0.05);且观察组水平均低于对照组(P<0.05);两组患者MMP-3、RANKL水平均下降,OPG水平增加(P<0.05),且观察组患者MMP-3、RANKL水平均低于对照组,OPG水平高于对照组(P<0.05)。结论独活寄生汤联合中药艾灸治疗RA肝肾亏虚证临床疗效较为显著,能降低RF、炎症反应、MMP-3、RANKL、OPG等预后不良因素对骨与关节的损伤。
Objective To observe the clinical efficacy of Duhuo Jisheng Decoction combined with TCM moxibustion in treatment of rheumatoid arthritis(RA)and its influene on matrix metalloproteinase-3(MMP-3),Nuclear factor-B receptor activator ligand(RANKL)and osteoprotegerin(OPG). Methods 68 RA patients were randomly divided into observation group and control group with 34 cases each. Control group was given diclofenac sodium capsules and methotrexate,and observation group was treated with Duhuo Jisheng Decoction combined with TCM moxibustion on the basis of control group. The total effective rate and rheumatoid factor(RF),Erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),MMP-3,RANKL and OPG of the two groups were compared. Results The total effective rate of treatment in observation group was higher than control group(P<0.05);after treatment,rheumatoid factor,erythrocyte sedimentation rate and CRP in both groups decreased,and were lower in observation group than control group(P<0.05);the levels of MMP-3,RANKL decreased,OPG increased in both groups(P<0.05),and the levels of MMP-3 and RANKL of observation group were lower than those of control group,and the OPG level was higher than that of control group(P<0.05). Conclusion Duhuo Jisheng Decoction combined with TCMmoxibustion has significant clinical effects on RA,which reduces the damage to bones and joints caused by unfavorable factors such as RF and inflammation.
