磁共振成像在脊髓室管膜瘤鉴别诊断中的应用及特征分析

摘要 目的探讨磁共振成像在脊髓室管膜瘤鉴别诊断中的应用及特征。方法选取2018年1~12月接受脊髓室管膜瘤治疗的54例患者,均进行磁共振扫描,对其扫描结果进行分析评估。结果54例患者中肿瘤主要分布于胸髓(33.3%),其次为颈髓(25.9%)及颈胸交界区(18.5%),而在胸腰交界区及腰髓的分布则相对较少,分别为7.4%、14.8%;主要累及1~11个椎体范围;54例肿瘤患者中,经平片扫描边界较为清楚,经TWI扫描主要呈低等信号,经T2WI扫描则主要以不同程度高信号为主;所有患者中存在瘤内囊变者36例(66.7%),继发性囊变者18例(33.3%);强化方式中呈均一强化者26例(48.1%),线状强化者22例(40.7%),无明显强化者6例(11.1%),与平扫比较强化后肿瘤的边界更加清晰。结论对脊髓内室管膜瘤患者采用磁共振成像技术,能够对其发病部位、边界及是否存在囊内瘤变等进行准确扫描,同时可见脊髓内室管膜瘤主要呈中心性生长,其边界相对清晰,且存在较为显著的均匀强化情况,临床医师可依据其上述特征进行临床诊断,从而有助于治疗方案的有效确立及实施。 Objective To investigate the application and characteristics of magnetic resonance imaging in differential diagnosis of spinal ependymoma.Methods A total of 54 patients with spinal ependymoma who were treated in our hospital from January 2018 to December 2018 were enrolled in the study.The magnetic resonance imaging(MRI)was performed for all the patients and the results were analyzed and evaluated.Results Of the 54 patients,the tumors were mainly located in thoracic spinal cord(33.3%),followed by the cervical spinal cord(25.9%)and the cervical thoracic junction(18.5%),which was mainly involved 1~11 vertebral range.Among the 54 patients,the boundary of plain film was clear,the main signal was low signal on TWI,and high signal in different degrees on T2WI.Of the patients,36 cases(66.7%)had intratumoral cystic change,18 cases(33.3%)had secondary cystic change,and 26 cases(48.1%)had homogeneous enhancement,22 cases(40.7%)had linear enhancement,6 cases(11.1%)had no obvious enhancement.As compared with that by plain scan,the boundary of enhanced tumor was more clear after the enhancement.Conclusion The application of MRI in patients with spinal ependymoma can scan accurately its lesion position and boundary,and evaluate whether there exists pouch neoplasia,which is helpful for clinicians to diagnose and treat according to the characteristics of imaging.
