1714例创伤标本分离的病原菌分布及耐药性分析 被引量:1

摘要 目的对本院骨科患者的1714例创伤标本进行病原菌分布及耐药性分析,更好地指导临床合理用药。方法对宁夏回族自治区人民医院2014年1月至2018年12月骨科及外伤患者的创伤标本按照规程进行分离培养,用VITEK-2细菌分析仪进行细菌鉴定,用K-B法及MIC法进行药敏监测,用WHONET 5.6统计软件对数据进行统计分析。结果此次回顾性分析了1821份阳性标本,去除重复菌株共有1714份菌株,革兰阴性菌859株,占50.1%;革兰阳性菌816株,占47.6%;真菌检出39例,占2.3%。其中革兰阴性菌主要为大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、阴沟肠杆菌、鲍曼不动杆菌等,革兰阳性球菌主要致病菌为金黄色葡萄球菌、屎肠球菌和粪肠球菌。根据药敏结果显示,革兰阴性杆菌除鲍曼不动杆菌外对碳青霉烯类药物最为敏感,阿米卡星耐药菌也较低,未发现耐药株,本研究中革兰阳性菌未发现对利奈唑胺、万古霉素、替考拉宁的耐药菌株。结论了解伤口组织感染的病原菌分布及耐药性特点,有助于临床合理用药,控制感染。 Objective The pathogen distribution and drug resistance isolated from 1714 wound specimens from orthopedics department in our hospital were analyzed to better guide clinical rational drug use.Methods The wound specimens of orthopedics and trauma patients from January 2014 to December 2018 in our hospital were isolated and cultured according to the regulations.The bacteria were identified by VITEK-2 bacteria analyzer,and the drug sensitivity monitoring was performed by K-B and MIC methods.Data were analyzed by WHONET 5.6 Software.Results A total of 1821 positive specimens were analyzed retrospectively,removal of a total of 1714 strains of repetitive strains,mainly Gram-negative bacteria,a total of 859 strains,accounting for 50.1%;816 strains of Gram-positive bacteria,accounting for 47.6%;39 cases of fungi were detected,accounting for 2.3%;of which Gram-negative bacteria were mainly Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa,Enterobacter cloacae,Acinetobacter baumannii,etc.,the main pathogen of Gram-positive cocci was gold Staphylococcus aureus,Enterococcus faecium and Enterococcus faecalis.According to the results of drug susceptibility,Gram-negative bacilli were most sensitive to carbapenems except for Acinetobacter baumannii,and were resistant to ertapenem and amikacin-resistant bacteria.No drug-resistant strains were found.In this study,no resistant strains to linezolid,vancomycin and telicoplanin were found.Conclusion Understanding the distribution and drug resistance of pathogens in wound tissue infection is helpful for rational drug use and control infection.
