478例糖尿病合并尿路感染患者的病原菌分布及耐药性分析 被引量:11

Distribution and Drug Resistance Analysis of Pathogens in 478 Patients with Mellitus Complicated with Urinary Tract Infection

摘要 目的探讨478例糖尿病合并尿路感染患者的病原菌分布及其耐药性,为临床控制糖尿病患者尿路感染用药提供依据。方法回顾性分析478例糖尿病合并尿路感染标本的病原菌,用VITEK-2细菌分析仪进行细菌鉴定,用K-B法或MIC法进行药敏监测。结果 478例感染的病原菌中,以革兰阴性菌为主,共检出350株,占73.2%;革兰阳性球菌101株,占21.1%;真菌27株,占5.7%。根据药敏结果显示,大肠埃希菌的耐药性相对高于肺炎克雷伯菌,但均未发现对亚胺培南和美洛培南耐药的菌株。三种主要的革兰阳性菌中未发现有对万古霉素、替考拉宁、利奈唑胺耐药的菌株,屎肠球菌对青霉素、左氧氟沙星、环丙沙星的耐药性均达到90%以上。结论糖尿病合并尿路感染患者分离的病原菌呈多样化,且耐药性强。 Objective The distribution and drug resistance of pathogens in 478 patients with diabetes mellitus complicated with urinary tract infection were studied,which will provide a basis for clinical control of urinary tract infection in diabetic patients.Methods The pathogens of 478 cases of diabetes mellitus complicated with urinary tract infection were retrospectively analyzed.The bacteria were identified by VITEK-2 bacteria analyzer.The drug sensitivity monitoring was performed by K-B or MIC method.Results Among the 478 infected pathogens,350 were Gram-negative bacteria,accounting for 73.2%;101 were Gram-positive bacteria,accounting for 21.1%;27 were fungi,accounting for 5.7%.According to the drug sensitivity results,the resistance of Escherichia coli was relatively higher than that of Klebsiella pneumoniae,but none of them were resistant to imipenem and meropenem.Among the three main Gram-positive bacteria,vancomycin,tiraconin and linezolid resistant strains were not found.The resistance of Enterococcus faecium to penicillin,levofloxacin and ciprofloxacin were all over 90%.Conclusion Pathogens isolated from patients with diabetes mellitus complicated with urinary tract infection are diverse and highly resistant.
