摘要
目的分析急性大脑中动脉闭塞患者MRI中Flair序列图像上高信号血管的范围与MRI灌注图像(PWI)参数的相关关系,探讨Flair高信号血管征(FHVs)在评估急性大脑中动脉闭塞患者侧支循环中的临床应用价值。方法回顾性分析38例急性大脑中动脉闭塞患者的Flair图像及PWI图像。对Flair图像上梗死区域血管呈高信号的范围进行评分及分析,计算梗死区域与对侧正常区域的PWI参数平均通过时间(MTT)、达峰时间(TTP)、造影剂到达时间(T0)的差值(△MTT、△TTP、△T0),将FHVs评分与PWI灌注参数差值进行相关分析。结果FHVs评分为(4.92±0.82),其中左、右侧大脑中动脉闭塞患者评分比较差异无统计学意义(P>0.05);低分组与高分组之间△MTT、△TTP比较,差异均存在统计学意义(均P<0.05)。相关性分析显示,FHVs评分与△TTP差异有统计学意义(P<0.05),FHVs评分与△TTP呈独立正相关;而与△MTT、△T0相关性分析差异无统计学意义(均P>0.05)。结论大脑中动脉急性闭塞的患者,FHVs评分与MR PWI中△TTP呈正相关,FHVs评分可提示梗死部位侧支循环的代偿能力。
Objective To analyze the correlation between the range of high-signal vessels in Flair images and perfusion weighted imaging(PWI)parameters and to investigate the clinical value of Flair hypersignal in the evaluation of collateral circulation in patients with acute middle cerebral artery occlusion.Methods Flair images and PWI images of 38 patients with acute middle cerebral artery occlusion were retrospectively analyzed.The range of hypersignal in the infarct area of the Flair image was scored,the differences of PWI parameters mean transit time(MTT),time to peak(TTP),T0(△MTT,△TTP,△T0)between the infarct area and the normal area of the contralateral side were calculated,and then the correlation between the score of FHVs and the difference of PWI perfusion parameters was analyze.Results FHVs score was(4.92±0.82).There was no significant difference in left and right middle cerebral artery occlusion scores(P>0.05);there were significant differences in△MTT,△TTP between low score and high score groups(allP<0.05).Correlation analysis showed that there was statistical significance between FHVs scores and△TTP(P<0.05),and there was an independent positive correlation between FHVs scores and△TTP,but there was no significant correlation between FHVs scores and△MTT or△T0(all P>0.05).Conclusions In patients with acute middle cerebral artery occlusion,FHVs scores are positively correlated with△TTP in PWI images.FHVs scores may indicate the compensatory ability of collateral circulation at infarct site.
