大脑中动脉闭塞FLAIR血管高信号征与CTA对比研究 被引量:12

Comparative study between FLAIR vascular hyperintensity sign and CTA in patients with middle cerebral artery occlusion

摘要 目的探讨FLAIR血管高信号征(FVH)对大脑中动脉闭塞(MCAO)患者软脑膜侧支循环的评估价值。方法对38例发病3天内的MCAO缺血性脑卒中患者的FVH和CTA图像进行对比分析。对FVH行FVH评分(纵向)[FVHS(L)]、FVH评分(横向)[FVHS(T)]两项评定,对CTA重建图像行侧支循环评分(纵向)[CS(L)]、侧支循环评分(横向)[CS(T)]两项评定。比较FVHS(L)与CS(L)、FVHS(T)与CS(T)评分相符程度,并分析FVHS(L)、FVHS(T)、CS(L)、CS(T)之间相关性。结果 38例MCAO患者中,20例(20/38,52.63%)FVHS(L)和CS(L)评价相符,21例(21/38,55.26%)FVHS(T)和CS(T)评价相符。FVHS(L)与CS(L)呈正相关(r=0.48,P=0.002),FVHS(T)与CS(T)呈正相关(r=0.54,P<0.001)。FVHS(L)与FVHS(T)呈正相关(r=0.34,P=0.034),CS(L)与CS(T)呈正相关(r=0.56,P<0.001)。结论对于MCAO患者,FVH是一种方便、经济、有效的软脑膜侧支循环评估方法。 Objective To evaluate the ability of FLAIR vascular hyperintensity(FVH)displaying leptomeningeal collateral circulation in patients with middle cerebral artery occlusion(MCAO).Methods The FVH and CTA images of 38 ischemic stroke patients with MCAO within 3days of onset were compared and analyzed.The FVH score(longitudinal direction)[FVHS(L)]and FVH score(transverse direction)[FVHS(T)]in T2-FLAIR were assessed.The collateral score(longitudinal direction)[CS(L)]and collateral score(transverse direction)[CS(T)]in CTA reconstructed image were assessed.The FVHS(L)and the CS(L)were compared,as well as the FVHS(T)and CS(T).The Spearmancorrelation coefficient was determined between the FVHS(L),FVHS(T)and CS(L),CS(T).Results Of the 38 MCAO patients,there were well correspondences with FVHS(L)and CS(L)(20/38,52.63%),FVHS(L)and CS(L)(21/38,55.26%).FVHS(L)were positively correlated with CS(L)(r=0.48,P=0.002).FVHS(T)were positively correlated with CS(T)(r=0.54,P〈0.001).FVHS(L)were positively correlated with FVHS(T)(r=0.34,P=0.034).CS(L)were positively correlated with CS(T)(r=0.56,P〈0.001).Conclusion FVH is a convenient,economic and valid evaluation method of leptomeningeal collateral circulation in patients with MCAO.
