摘要
目的探讨彩色多普勒超声检查小儿常见急腹征的诊断价值。方法分析200例急腹征患儿的彩色多普勒超声资料,以临床随诊、术中探查和术后病检结果为金标准,观察彩色多普勒超声对不同类型急腹征的诊断效果。结果金标准诊断结果为肠套叠11例,肠梗阻35例,肠系膜淋巴结炎74例,急性阑尾炎80例。超声诊断结果为肠套叠11例,肠梗阻33例,肠系膜淋巴结炎74例,急性阑尾炎72例,其符合率分别为100.00%、94.29%、100.00%、90.00%。超声诊断肠梗阻结果表明,小肠梗阻和结肠阻梗符合率分别为95.83%、90.91%。超声诊断急性阑尾炎结果表明,单纯性阑尾炎、化脓性阑尾炎、周围脓肿阑尾炎和坏疽性急性阑尾炎的符合率分别为100.00%、87.50%、83.33%和72.73%。结论彩色多普勒超声检查对小儿常见急腹征临床诊断效果较好,为临床治疗提供依据。
Objective To explore the diagnostic value of color Doppler ultrasonography for infants with common acute abdomen.Methods The color Doppler ultrasound data of 200 infants with acute abdomen was analyzed.Taking clinical follow-up,intraoperative exploration or postoperative disease test results as gold standard,the effects of color Doppler ultrasonography in diagnosis of different acute abdominal diseases were observed.Results The gold standard diagnosis results showed intussusception in 11 cases,intestinal obstruction in 35 cases,mesenteric lymphadenitis in 74 cases and acute appendicitis in 80 cases.The ultrasonic diagnosis results showed intussusception in 11 cases,intestinal obstruction in 33 cases,mesenteric lymphadenitis in 74 cases and acute appendicitis in 72 cases,their coincidence rates of intussusception,ileus,mesenteric lymphadenitis and acute appendicitis were 100.00%,94.29%,100.00%and 90.00%,respectively.The ultrasonic diagnosis results of intestinal obstruction showed that the coincidence rates of small intestinal obstruction and colon obstruction were 95.83%and 90.91%,respectively.The ultrasonic diagnosis results of acute appendicitis showed that the coincidence rates of simple,suppurative,peripheral abscess and gangrene acute appendicitis were 100.00%,87.50%,83.33%and 72.73%,respectively.Conclusion Color Doppler ultrasonography has a good effect in diagnosis of acute abdomen in infants,which provides a basis for clinical treatment.
