Practical animal model of emergency airway training for war wounds

摘要 目的建立战伤现场紧急气道管理培训动物模型,供战伤现场救治人员学习气道管理技术,提高战伤现场救治气道管理能力。方法枪弹伤猪失血性动物模型建立观察完成后,选取猪舌、会厌、声门、甲状软骨、环甲膜、环状软骨、气管、肺等完整的猪呼吸系统解剖作为培训模型,利用完整的猪的气道进行战伤现场紧急气道管理技能培训。根据标准操作流程进行喉罩、喉通气管、食管气管联合导管以及气管插管的培训。选取25名具有丰富气道管理技能培训经验的教员,分别使用不同气道工具在气道培训动物模型以及气道管理模型(Ambu)进行操作比较。结果采用猪作为动物模型进行战伤现场紧急气道管理培训,在操作时间上,除喉罩外,喉通气管、食管气管联合导管以及气管插管的置管成功时间明显少于气道管理模型所用时间,同时所有操作者均表示使用动物模型能够更直观形象模拟不同气道工具放置后的通气效果。结论猪气道解剖同人体结构类似,能够真实模拟战伤现场救治中紧急气道管理培训的通气效果。可为战伤现场救治人员的教学与技能培训提供实用的动物模型。 Objective To establish an animal model for emergency airway management training at the site of war injuries,so that the rescue personnel can learn airway management techniques and improve the ability of airway management at the site of war injuries.Methods After the establishment and observation of the pig hemorrhagic animal model with gunshot wounds,the complete pig respiratory system anatomy including pig tongue,epiglottis,glottis,thyroid cartilage,cricothyroid membrane,ring cartilage,trachea,lung and others was selected as the training model.Use the complete pig airway to conduct emergency airway management skills training on the battlefield.Training on laryngeal mask,laryngeal lube,esophagotracheal combined catheter and tracheal intubation should be carried out according to the standard operation procedure.25 instructors with rich training experience in airway management skills were selected and compared using different airway tools in the airway training animal model and airway management model(Ambu).Results In terms of operation time,except for the laryngeal mask,the successful intubation time of the laryngeal tube,esophagotracheal combined catheter and tracheal intubation was significantly shorter than that of airway management model.At the same time,all operators indicated that the animal model could simulate the ventilation effect of different airway tools more intuitively and vividly.Conclusion The anatomy of pig airway is similar to that of human body and it can truly simulate the ventilation effect of emergency airway management training at the site of war injuries.It can provide a practical animal model for the teaching and skill training of rescue personnel at the site of war injuries.
