摘要
目的:探讨热牙胶垂直加压法在牙体牙髓病患者中的疗效观察及对咀嚼功能的影响。方法:选择2018年1月~2019年12月牙体牙髓病患者290例作为对象,按照选择治疗方式的不同分为对照组(n=145例)和观察组(n=145例)。对照组给予冷牙胶侧方加压根管填充治疗,观察组采用热牙胶垂直加压法根管填充治疗。治疗后对患者治疗效果进行评估,比较两组咀嚼效能及基牙相关指标、相关因子变化、疼痛评分及填充效果。结果:观察组治疗后咬合力、咀嚼效能高于对照组,牙龈指数、出血指数低于对照组(P<0.05);观察组治疗后血清CRP、IL-6、IL-2水平低于对照组(P<0.05)。观察组治疗后3周疼痛评分低于对照组;观察组的恰填率显著高于对照组,欠填和超填率显著低于对照组(P<0.05)。结论:热牙胶垂直加压法在牙体牙髓病患者中应用效果显著,根尖填充密度良好,有助于提高患者的咀嚼效能,缓解患者的疼痛,调节相关炎症因子的水平。
Objective:To investigate the effect of vertical compression of hot tooth gum on patients with dental pulp disease and its effect on chewing function.Methods:From January 2018 to December 2019,290 patients with dental pulp disease were selected as subjects.They were divided into a control group(n=145 cases)and an observation group(n=145 cases)according to different treatment methods.The control group was given cold tooth gum lateral compression root canal filling treatment,and the observation group with hot tooth gum vertical compression root canal filling treatment.After treatment, the chewing efficiency and abutment related indexes, changes of related factors, pain score and filling effect between the two groups were compared.Results:The bite force and chewing efficiency of the observation group after treatment were higher than those of the control group,the gingival index and bleeding index of the observation group after treatment were lower(P<0.05).The serum CRP and IL-6 of the observation group after treatment The level of IL-2 was lower than that of the control group(P<0.05).The pain score of the observation group was lower than that of the control group 3 weeks after treatment. The exact filling rate of the observation group was significantly higher than the control group,and the underfilling and overfilling rates were significantly lower(P<0.05).Conclusion:The application of vertical compression of hot tooth gum in patients with dental pulp disease has a significant effect,and the apical filling density is good,which helps to improve the chewing efficiency of patients,relieve patients’ pain,and adjust the level of related factors.
