摘要
目的分析男性不育患者年龄与精子DNA碎片率和精液常规参数的相关性。方法选择2018年1~12月在本院男科就诊的患者725例,按照年龄将患者分为三组。20~29岁组(n=229),30~39岁组(n=371),40~49组(n=125),采用染色质扩散(SCD)实验检测精子DNA碎片率,按照《世界卫生组织人类精液检查与处理实验室手册》(第5版)中标准检测精液常规各参数。分析精子DNA碎片率(DNA fragmentation index,DFI)和精子参数在各组的差异及年龄与精子DNA碎片率和精液参数的相关性。结果年龄与DFI之间存在正相关关系(r=0.273,P<0.01),与精子活力(r=-0.131,P<0.01)、精液pH(r=-0.080,P<0.05)之间存在负相关关系,与精子浓度之间的相关性差异无统计学意义。精液pH与精子浓度之间存在正相关关系(r=0.285,P<0.05)。结论伴随着男性年龄的增长,精子的活力会降低,并使男性的生育力下降以及对子代产生不良影响,精子DFI与精液常规各参数无相关性。故精子DNA碎片检测可作为精液常规分析的补充,进一步反映男性的生育能力。
Objective To analyze the correlation between the age of male infertile patients and sperm DNA fragmentation index(DFI)and the routine parameters of semen.Methods 725 patients treated in Department of Andrology of our hospital from January to December 2018 were selected and divided into three groups according to their age.For the group at the age of 20-29(n=229),the group at the age of 30-39(n=371),and the group at the age of 40-49(n=125),the sperm DFI was detected through sperm chromatin dispersion(SCD)test,and the various routine semen parameters were detected according to the standards in"WHO laboratory manual for the examination and processing of human semen"(The 5 th Edition).The differences of sperm DFI and sperm parameters among the groups and the correlation between age and sperm DFI and the semen parameters were analyzed.Results There was a positive correlation between age and DFI(r=0.273,P<0.01),and a negative correlation between age and sperm motility(r=-0.131,P<0.01),age and the semen pH(r=-0.080,P<0.05),and the difference in the correlation between age and sperm concentration was statistically insignif icant.There was a positive correlation between the semen pH and sperm concentration(r=0.285,P<0.05).Conclusion With the increase of the age of men,sperm motility may decrease,affecting male fertility.Sperm DFI has no correlation with the various routine semen parameters.Therefore,the detection of sperm DNA fragments test can be used as a supplement to routine semen analysis,further reflecting the male fertility.
