摘要
目的探讨利拉鲁肽、二甲双胍联合短期持续正压通气治疗2型糖尿病(T2DM)伴阻塞性睡眠呼吸暂停综合征(OSAHS)的临床疗效。方法选取唐山市协和医院2017年12月至2019年12月收治的T2DM伴OSAHS患者106例,随机分为观察组和对照组,各53例。两组患者均于夜间睡眠时予持续正压通气,并口服二甲双胍片,观察组患者加用利拉鲁肽注射液皮下注射,均持续治疗3个月。结果治疗后,观察组患者的空腹血糖(FBG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1C)、丙二醛(MDA)、8-异前列腺素(8-iso-PG)水平及胰岛素抵抗指数(HOMA-IR)、呼吸暂停-低通气指数(AHI)均显著低于对照组(P<0.05),血氧饱和度(SpO2)、血氧分压(PO2)及超氧化物歧化酶(SOD)水平均显著高于对照组(P<0.05);观察组与对照组不良反应发生率相当(16.98%比11.32%,P>0.05)。结论利拉鲁肽、二甲双胍联合短期持续正压通气治疗T2 DM伴OSAHS,能有效控制血糖水平,改善通气功能障碍,降低机体氧化应激水平。
Objective To study the effect of lilalutide+metformin combined with short-term continuous positive pressure ventilation on the therapeutic effect and oxidative stress index of type 2 diabetes mellitus(T2 DM)patients with obstructive sleep apnea syndrome(OSAHS).Methods A total of 106 T2 DM patients with OSAHS admitted to Tangshan Union Hospital from December 2017 to December 2019 were selected and randomly divided into the observation group and control group,53 cases in each group.Both groups were given short-term continuous positive pressure ventilation treatment during sleep at night and metformin tablet oral treatment;the observation group was combined with lilalutide treatment on this basis.All patients were continuously treated for 3 months.Results After treatment,the levels of fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPG),glycosylated hemoglobin(Hb A1 C),malondialdehyde(MDA),and 8-isoprostaglandin(8-iso-PG),insulin resistance index(HOMA-IR),apnea-hypopnea index(AHI)in the observation group were significantly lower than those of the control group(P<0.05);the levels of blood oxygen saturation(SpO2),blood oxygen partial pressure(PO2)and superoxide dismutase(SOD)were significantly higher than those of the control group(P<0.05);the incidence of adverse reactions in the observation group and the control group was equivalent(16.98%vs.11.32%,P>0.05).Conclusion Liraglutide+metformin combined with short-term continuous positive pressure ventilation for patients with T2 DM with OSAHS can effectively control the blood glucose level,improve ventilation dysfunction,and reduce the body’s oxidative stress level.
