摘要
目的:比较不同能量的Er:YAG激光对早期种植体周围炎治疗的临床疗效。方法:纳入90例(男51例,女39例,年龄24~69岁)早期种植体周围炎患者,分A、B、C、D四组,治疗前均进行传统的龈上洁治及龈下手工刮治。实验组分别用40 mJ、60 mJ、80 mJ的Er:YAG激光进行照射,术中用视觉模拟评分法(visual analogue scale,VAS)记录患者疼痛程度;对照组用盐酸米诺环素处理牙周袋。临床治疗1次/周,3次后记录第1周、4周、8周、12周时各组临床指标:菌斑指数(plaque index,PLI)、出血指数(bleeding index,BI)、探诊深度(probing depth,PD)、骨缺损深度(defect depth,DD),并与基线指标进行比较。结果:治疗后各组临床指标较治疗前均有明显改善(P<0.05)。PLI在各组无明显差异;BI的改善在80 mJ组优势突出;第12周时实验组PD及DD改善优于对照组(P<0.05);在PD改善上第1周时40 mJ较其他组弱,第4周、8周及12周时60 mJ、80 mJ的改善突出;术中VAS比较能量越大患者的评分越高。结论:在安全范围内,本研究中推荐使用60 mJ、20 Hz的Er:YAG激光对早期种植体周围炎进行照射,效果较佳。
Objective: to compare the clinical efficacy with different energy of Er:YAG laser in the treatment of early peri-implantitis. Methods: Total 90 patients(51 males and 39 females, aged 24~69 years old) with early peri-implantitis were enrolled, divided into A, B, C and D groups, the traditional sputum scaling and underarm treatment were performed before all the treatments.The experimental group was irradiated with Er:YAG laser of 40 mJ, 60 mJ and 80 mJ respectively, and recording the value of VAS during operation.Meanwhile,the control group was treated with minocycline hydrochloride for implant-periodontal pockets. Treat for 1 time/week, total 3 weeks,after that recording each group’s clinical indicators(plaque index PLI, bleeding index BI, probing depth PD, bone defect depth DD) for the 1 st week, 4 th week, 8 th week and 12 th week,and comparing with baseline indicators. Results: the clinical indexes of each group were significantly improved after treatment(P<0.05). There was no significant difference in PLI between all groups;the improvement of BI was prominent with 80 mJ laser;the improvement of PD and DD in the experimental group was better than that in the control group at the 12 th week(P<0.05);40 mJ in the 1 st week of PD improvement was weaker than the others,but the improvement of 60 mJ and 80 mJ was prominent at the 4 th, 8 th and 12 th week;the higher the energy of the intraoperative VAS, the higher the score of the patient. Conclusion: in the safe range for this study, 60 mJ, 20 Hz Er:YAG laser is recommended for better efficacy while treating early peri-implantitis.
