重型颅脑损伤术后颅内感染因素及干预建议效果 被引量:3

摘要 目的探讨重型颅脑损伤术后颅内感染因素及干预方法。方法通过对2017年1月—2019年9月在该院治疗40例重症颅脑损伤颅内感染患者临床资料分析,分析术后颅内感染的因素,为临床治疗、护理提出建议。结果单因素分析,年龄≥50岁、手术次数≥2次、手术时间≥4 h、有合并症、术前GCS评分<8分、糖蛋白含量>1000 mg/L、引流管拔管时间≥3 d、脑室外引流、术后白蛋白≥35 g/L、抗菌药物使用时间≥14 d、术后未经亚低温治疗、切口脑脊液漏均是引起重型颅脑损伤术后颅内感染的单因素(χ^2=6.744、5.839、6.305、7.374、8.772、7.183、9.654、10.592、4.617、6.345、12.975、7.247,P<0.05);经Logistic回归分析,高龄、术前GCS评分<8分、切口脑脊液漏、糖蛋白含量>1000 mg/L、低蛋白血症、引流管拔管时间≥3 d均为颅内感染独立危险因素(OR=1.256、3.477、6.926、5.032、4.021、3.000,P<0.05)。结论多种因素同重症颅脑损伤患者术后颅内感染相关,针对颅内感染因素采取积极预防措施,降低术后颅内感染发生率。 Objective To explore the factors and intervention methods of intracranial infection after severe head injury.Methods Through the analysis of clinical data of 40 patients with severe craniocerebral injury intracranial infection treated in our hospital from January 2017 to September 2019,the factors of intracranial infection after operation were analyzed,and recommendations were made for clinical treatment and nursing.Results Single factor analysis,age≥50 years old,number of operations≥2,operation time≥4 h,comorbidities,preoperative GCS score<8 points,glycoprotein content>1000 mg/L,drainage tube extubation time≥3 d,brain outdoor drainage,postoperative albumin≥35 g/L,antibacterial drug use time≥14 d,postoperative hypothermia treatment,incision cerebrospinal fluid leakage were all single factors causing intracranial infection after severe craniocerebral injury(χ^2=6.744,5.839,6.305,7.374,8.772,7.183,9.654,10.592,4.617,6.345,12.975,7.247,P<0.05);Logistic regression analysis,advanced age,preoperative GCS score<8 points,incision cerebrospinal fluid leakage,glycoprotein content>1000 mg/L,hypoproteinemia,drainage tube extubation time≥3 d are independent risk factors for intracranial infection(OR=1.256,3.477,6.926,5.032,4.021,3.000,P<0.05).Conclusion Various factors are associated with postoperative intracranial infection in patients with severe craniocerebral injury.Active preventive measures should be taken for intracranial infection factors to reduce the incidence of postcranial infection.
