超声引导下行肌间沟臂丛神经阻滞对上肢手术患者麻醉效果及血流动力学的影响 被引量:10

摘要 目的:探讨超声引导下行肌间沟臂丛神经阻滞对上肢手术患者麻醉效果及血流动力学的影响。方法:选取本院2019年1-12月进行上肢手术治疗的60例患者的临床资料进行回顾性分析。根据手术麻醉方式不同分为对照组(n=30)与观察组(n=30)。对照组给予传统肌间沟臂丛神经阻滞,观察组给予超声引导下行肌间沟臂丛神经阻滞。比较两组麻醉效果相关指标、不同时间点血流动力学及不良反应发生情况。结果:观察组麻醉完成时间和起效时间均短于对照组,且神经阻滞效果评分低于对照组(P<0.05)。麻醉前与术毕30 min时,两组心率(HR)与舒张压(DBP)比较,差异均无统计学意义(P>0.05);手术开始15、45 min时,观察组HR与DBP水平均高于对照组(P<0.05)。麻醉前,两组收缩压(SBP)比较,差异无统计学意义(P>0.05);手术开始15、45 min与术毕30 min时,观察组SBP均高于对照组(P<0.05);麻醉前、手术开始15、45 min与术毕30 min时,两组血氧饱和度(SpO2)比较,差异均无统计学意义(P>0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:超声引导下行肌间沟臂丛神经阻滞可缩短上肢手术患者麻醉药起效时间,提高麻醉效果,有效稳定患者麻醉期间血流动力学,安全性较高。 Objective:To investigate the effect of ultrasound-guided interscalene brachial plexus block on anesthesia and hemodynamics in patients undergoing upper limb surgery.Method:The clinical data of 60 patients undergoing upper extremity surgery in our hospital from January to December 2019 were retrospectively analyzed.According to different anesthesia methods,they were divided into control group(n=30)and observation group(n=30).The control group was given traditional interscalene brachial plexus block,and the observation group was given ultrasound-guided interscalene brachial plexus block.The related indexes of anesthesia effect,hemodynamics at different time points and adverse reactions were compared between the two groups.Result:The completion time and onset time of anesthesia in the observation group were lower than those in the control group,and the score of nerve block effect was lower than that in the control group(P<0.05).Before anesthesia and 30 min after surgery,there were no significant differences in heart rate(HR)and diastolic blood pressure(DBP)between the two groups(P>0.05).At 15 and 45 min after surgery began,HR and DBP levels in the observation group were higher than those in the control group(P<0.05).Before anesthesia,there was no significant difference in systolic blood pressure(SBP)between the two groups(P>0.05).At 15,45 min after surgery began and 30 min after surgery,SBP in the observation group was higher than that in the control group(P<0.05).Before anesthesia,15,45 min after surgery began and 30 min after surgery,there were no significant differences in blood oxygen saturation(SpO2)between the two groups(P>0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Ultrason-guided interscalene brachial plexus block can shorten the onset time of anesthetics in patients undergoing upper limb surgery.It can improve the anesthetic effect,effectively stabilize the hemodynamics during anesthesia and have higher safety.
