摘要
目的系统评价父亲年龄≥40岁对不孕夫妇体外受精-胚胎移植(In vitro fertilization-embryo transfer,IVF-ET)治疗后妊娠结局的影响。方法通过计算机检索Pub Med、EMBASE、Web of Science、中国知网、万方数据库、维普数据库和中国生物医学文献数据库,收集国内外有关父亲年龄≥40岁对不孕夫妇体外IVF-ET术后妊娠、流产和活产影响的研究,采用Rev Man 5.3软件进行Meta分析。结果纳入文献11篇,其中包含父亲年龄≥40岁组872例,父亲年龄<40岁组5322例。Meta分析结果显示父亲年龄≥40岁组的妊娠率(OR=0.80,95%CI:0.69-0.93,P=0.004)、生化妊娠率(OR=0.75,95%CI:0.57-0.99,P=0.04)和临床妊娠率(OR=0.74,95%CI:0.61-0.91,P=0.004)显著低于父亲年龄<40岁组,其流产率显著高于年龄<40岁组(OR=1.62,95%CI:1.05-2.51,P=0.03),在活产率方面两组无差异(OR=0.74,95%CI:0.40-1.37,P=0.34)。结论父亲年龄≥40岁会降低不孕夫妇IVF-ET术后妊娠率,增加流产率,但是不影响活产率。
Objective To systematically evaluate the effect of father’s age≥40 years old on the pregnancy outcome of infertile couples after in vitro fertilization embryo transfer(IVF-ET)treatment.Methods The related literatures on the effects of father’s age≥40 years old on pregnancy,abortion and live birth of infertile couples after IVF-ET treatment were searched and collected through the database of Pub Med,EMBASE,Web of Science,CNKI,CBM,VIP and Wanfang.The Rev Man 5.3 software was used for meta-analysis.Results Eleven related literatures were obtained,including 872 cases in the father’s age≥40 years old group and 5322 cases in the father’s age<40 years old group.The results of meta-analysis showed that the pregnancy rate(OR=0.80,95%CI:0.69-0.93,P=0.004),biochemical pregnancy rate(OR=0.75,95%CI:0.57-0.99,P=0.04)and clinical pregnancy rate(OR=0.74,95%CI:0.61-0.91,P=0.004)in the father’s age≥40 years old group were significantly lower than those in the father’s age<40 years old group.And the abortion rate in the father’s age≥40 years old group was significantly higher than that in the father’s age<40 years old group(OR=1.62,95%CI:1.05-2.51,P=0.03).There was no difference regards to live birth rate between two groups(OR=0.74,95%CI:0.40-1.37,P=0.34).Conclusion Our results suggest that paternal age over 40 years old may decrease the pregnancy rate and increase abortion rate in infertile couples after IVF-ET treatment,but has little effect on live birth rate.
