摘要
从叙事医学框架下对一则临床病例的细读,分析了医患不同的叙事视角,发现了除疾病以外,患者的心理、经济、信仰、社会关系等因素对治疗方案选择的影响。因此,医生在关注作为客体和实体存在的疾病的同时,也须考虑患者由于病痛所产生的主观情绪以及由于疾病而导致的社会关系的改变。在医生寻求延长患者生命的治疗方法时,患者不得不同时考虑其生活的质量与成本。由于视角的差异,医患对同一事物的理解也大相径庭。文本细读法为医患沟通搭建桥梁,为弥合医患分歧提供可能。
This article closely reads a clinical case in light of narrative medicine.The analysis of the differences between the patient's view and that of the doctor's leads to the discovery that apart from the disease itself,patients'psychological and economic conditions as well as their religious beliefs and social relations also play important roles in their choice of a treatment plan.Therefore,doctors need to take into account not only the diseases,but also patients'subjective feelings and the changes in their social relations as a result of the illness.While doctors try to seek ways to prolong the life of patients,patients also have to consider the quality and cost of their lives.Due to different perspectives,doctors and patients can have divergent understandings towards the same thing.Close reading helps smoothen doctor-patient communication and makes it possible to bridge the gaps between doctors and patients.
