骨科医院铜绿假单胞菌感染的分布及耐药性分析 被引量:1

Clinical distribution and drug resistance of Pseudomonas aeruginosa infection in orthopaedic hospital

摘要 目的探讨骨科医院铜绿假单胞菌感染的标本分布及药敏情况,为指导临床合理使用抗菌药物,控制医院感染提供科学依据。方法对骨科医院2015年1月-2018年12月铜绿假单胞菌感染的临床现状进行回顾性调查,统计分析452株铜绿假单胞菌的药敏试验结果。结果骨科医院铜绿假单胞菌的主要来源为伤口分泌物、脓液和呼吸道标本,分别占总数的46.9%、21.7%和16.8%;骨髓炎科、脊柱外科、ICU及骨盆创伤外科分离菌株最多,合计达385株,占85.2%;铜绿假单胞菌对氨苄西林/舒巴坦、庆大霉素、头孢他啶及头孢吡肟的耐药率均>70.0%,对其他抗菌药物也产生了不同的耐药性;对阿米卡星、哌拉西林、头孢哌酮及美罗培南的敏感性均>80.0%。结论骨科医院临床医师应根据患者细菌学检查结果,合理应用抗菌药物,采取有效措施积极预防和控制铜绿假单胞菌院内感染。 Objective To investigate the specimen distribution and drug-resistance in nosocomial infections with Pseudomonas aeruginosa in orthopaedic hospital,so as to provide the scientific evidence for clinical controlling of them.Methods The clinical status of nosocomial infections with Pseudomonas aeruginosa in orthopaedic hospital from Jan.2015 to Dec.2018 was investigated retrospectively.The susceptibility testing results of 452 cases were analyzed statistically.Results The specimen were mostly from wound secretions,pus,and respiratory tract(46.9%,21.7%,16.8%).Osteomyelitis,spinal surgery,ICU and pelvic trauma surgery were the most isolated strains,totaling 385 strains(85.2%).The drug-resistance rates of Pseudomonas aeruginosa to ampicillin/sulbactam,gentamicin,ceftazidime and cefepime were all above 70.0%,also to other antibiotics.The rates of susceptibility to amikacin,piperacillin,cefoperazone and meropenem were all above 80.0%.Conclusion The clinical should prescribe the antibiotics reasonably according to the results of bacteriological examination in orthopaedic hospital,take effective measures to prevent and control the positive Pseudomonas aeruginosa infection.
