摘要
目的分析头颈部腺样囊性癌高级别转化的临床特征及预后情况。方法采用病例回顾性分析,收集北京同仁医院2017年1月~2018年12月就诊并且病理明确诊断为腺样囊性癌的患者101例。依据高级别转化组织学形态诊断标准,选出6例高级别转化病理类型,收集整理其临床特征并完善随访信息,比较复发及转移、无病生存期等预后情况。结果6例腺样囊性癌高级别转化患者,随访时间为11~121个月,存活4例,死亡2例,平均无病生存期为29.8个月。6例中有5例均发生了局部复发及远处转移。结论高级别转化类型腺样囊性癌预后较差,复发及远处转移速度较快。
OBJECTIVE To analyze the clinical features and prognosis of high-grade transformation of adenoid cystic carcinoma.METHODS A retrospective analysis of cases was used to collect 101 patients who were treated in Beijing Tongren Hospital from January 2017 to December 2018 and whose pathological diagnose were adenoid cystic carcinoma.According to the high-grade transformation histological diagnostic criteria proposed by Seethala et al,six cases of high grade transformation pathology were selected from 101 patients.We collected clinical characteristics of six cases and improved the followup information.The prognose including recurrence and metastasis and disease-free survival time were compared.RESULTS The paper found that four of six patients with high-grade transformation survived and two of them died during the follow-up time lasting from 11months to 121 months.The average disease-free survival was 29.8 months.Local recurrence and distant metastasis occurred in 5 of 6 cases.CONCLUSION Adenoid cystic carcinoma high grade transformation has a poor prognosis,and the rate of recurrence and distant metastasis is fast.
