尿液微量蛋白检测甲状腺功能失调早期肾损害的诊断价值 被引量:2

摘要 目的探讨检测甲状腺功能异常患者尿液中的微量白蛋白(U-mAlb)、转铁蛋白(U-TRF)、α1-微球蛋白(U-α1-MG)和免疫球蛋白(IGU)水平的临床价值。方法选择2017年1月至2017年12月于本院住院后确诊的甲状腺功能失调症患者121例,其中甲状腺功能亢进患者78例(甲亢组)、甲状腺功能减退患者43例(甲减组),选择同期于本院健康体检者30名为对照组。检测甲状腺功能亢进患者(甲亢组)、甲状腺功能减退患者(甲减组)及健康者(对照组)尿液中的U-mAlb、U-TRF、U-α1-MG和IGU水平,比较各组治疗前后指标的变化。结果甲亢组与对照组U-mAlb、U-TRF、U-α1-MG和IGU水平比较差异具有统计学意义(P<0.05),治疗前后比较差异具有统计学意义(P<0.05)。甲减组与对照组U-mAlb、U-TRF、U-α1-MG水平比较差异具有统计学意义(P<0.05),治疗前后比较差异具有统计学意义(P<0.05)。甲亢组与甲减组U-α1-MG、IGU比较差异具有统计学意义(P<0.05)。结论检测甲状腺功能异常患者尿液中的U-mAlb、U-TRF、U-α1-MG和IGU水平可早期诊断患者的肾脏损伤,对评估治疗效果有一定的价值。 Objective To explore the clinical value of detecting urinary microalbumin(U-mAlb),transferrin(U-TRF),α1-microglobulin(U-α1-MG)and immunoglobulin(IGU)in patients with abnormal thyroid function.Methods A total of 121 patients with thyroid dysfunction diagnosed in our hospital from January 2017 to December 2017 were selected,including 78 patients with hyperthyroidism(hyperthyroidism group)and 43 patients with hypothyroidism(hypothyroidism group).30 healthy people who underwent physical examination in our hospital in the same period were selected as the control group.The levels of U-mAlb,U-TRF,U-α1-MG and IGU in urine of patients with hyperthyroidism(hyperthyroidism group),hypothyroidism patients(hypothyroidism group)and healthy people(control group)were measured,and the changes of indexes before and after treatment in each group were compared.Results There was significant difference in U-mAlb,U-TRF,U-α1-MG and IGU between the hyperthyroidism group and the control group(P<0.05),and there was significant difference before and after treatment(P<0.05).There was significant difference in the levels of U-mAlb,U-TRF and U-α1-MG between the hypothyroidism group and the control group(P<0.05),and there was significant difference before and after treatment(P<0.05).There was significant difference in U-α1-MG and IGU between hyperthyroidism group and hypothyroidism group(P<0.05).Conclusion Detection of urinary U-mAlb,U-TRF,U-α1-MG and IGU levels in patients with abnormal thyroid function can early diagnose renal injury of the patients,and has a certain value in evaluating the effect of treatment.
