摘要
目的:探讨选择性COX-2抑制剂与非甾体抗炎药预防肘部骨折术后异位骨化的临床效果。方法:根据不同的预防药物,将80例肘部骨折患者分为对照组与观察组,每组各40例。对照组给予非甾体抗炎药预防,观察组给予选择性COX-2抑制剂预防。比较两组术后异位骨化分型、肘关节功能、术后疼痛程度及不良反应发生情况。结果:观察组异位骨化发生率低于对照组(2.50%vs.20.00%,P<0.05)。两组异位骨化Hastings-Graham分型比较,差异无统计学意义(P>0.05)。治疗后,两组Mayo评分均高于治疗前,观察组Mayo评分高于对照组(P<0.05)。治疗7 d后,两组VAS评分显著低于治疗前(P<0.05)。观察组患者在治疗过程中不良反应总发生率为7.50%,低于对照组的30.00%(P<0.05)。结论:选择性COX-2抑制剂可显著减少肘部骨折术后异位骨化发生率,改善肘关节功能,减轻术后疼痛程度,降低不良反应发生率。
Objective:To observe the clinical effect of selective COX-2 inhibitors and non-steroidal anti-inflammatory drugs in preventing postoperative heterotopic ossification of elbow fracture.Methods:80 patients with elbow fractures were divided into control group and observation group according to different preventive drugs,40 cases in each group.The control group was given non-steroidal anti-inflammatory drugs for prevention,and the observation group was given selective COX-2 inhibitor.The postoperative heterotopic ossification,the elbow function,postoperative pain and adverse reactions of the elbow joints were compared between the two groups.Results:The incidence of heterotopic ossification in the observation group was lower than that in the control group(2.50%vs.20.00%,P<0.05).There was no significant difference in the heterotopic ossification Hastings-Graham classification(P>0.05).After treatment,the Mayo scores of the two groups were higher than before the treatment,and the Mayo scores of the observation group was higher than that of the control group(P<0.05).After 7 days of treatment,the VAS score was significantly lower than that before treatment(P<0.05).The total incidence of adverse reactions in the observation group was 7.50%,which was lower than the total incidence of adverse reactions in the control group(30.00%,P<0.05).Conclusion:Selective COX-2 inhibitor can significantly reduce the incidence of heterotopic ossification after elbow fracture,improve elbow joint function,reduce postoperative pain and reduce the incidence of adverse reactions.
