体外冲击波疗法联合常规康复治疗对肘关节骨折术后患者肘关节功能障碍的改善效果研究 被引量:2

Study on the Improvement Effect of Extracorporeal Shock Wave Therapy Combined with Conventional Rehabilitation Treatment on Elbow Joint Dysfunction in Patients after Elbow Joint Fracture Surgery

摘要 目的探讨体外冲击波疗法(ESWT)联合常规康复治疗对肘关节骨折术后患者肘关节功能障碍的改善效果.方法选择该院2019年1月—2022年3月收治的60例肘关节骨折术后患者为研究对象,随机分为对照组和试验组.对照组(n=30)采用常规康复治疗,试验组(n=30)在对照组基础上采用ESWT.比较两组肘关节功能、临床疗效以及肘关节活动度.结果治疗前,两组改良肘关节美国纽约特种外科医院(HSS)评分比较,差异无统计学意义(P>0.05);治疗后,两组患者改良肘关节HSS评分均高于同组治疗前,差异有统计学意义(P<0.05);试验组改良肘关节HSS评分(69.03±15.88)分高于对照组的(64.43±16.34)分,但差异无统计学意义(P>0.05).试验组总有效率高于对照组,但差异无统计学意义(P>0.05).治疗前,两组肘关节屈曲、伸展主动关节活动度(AROM)比较,组间差异无统计学意义(P>0.05);治疗后,试验组肘关节屈曲AROM(76.17±10.74)°明显较对照组的(68.43±14.58)°增大,伸展AROM(-0.70±1.42)°明显较对照组的(-1.23±2.01)°减小,组间差异有统计学意义(P<0.05).结论单纯常规康复治疗与ESWT联合常规康复治疗均可对肘关节骨折术后患者发挥较好的治疗作用,但ESWT联合常规康复治疗在改善肘关节屈曲、伸展功能方面的效果更加显著. Objective To explore the improvement effect of extracorporeal shock wave therapy(ESWT)combined with conventional rehabilitation treatment on elbow joint dysfunction in patients after elbow joint fracture surgery.Methods 60 patients with elbow joint fracture treated in the hospital from January 2019 to March 2022 were selected as the study subjects,and were randomly divided into a control group and an experimental group.The control group(n=30)received conventional rehabilitation treatment,and the experimental group(n=30)received ESWT on the basis of the control group.The elbow joint function,clinical efficacy and range of motion of elbow joint were compared between the two groups.Results Before treatment,there was no significant difference in hospital for special surgery(HSS)scores of the modified elbow joint between the two groups(P>0.05);after treatment,the modified elbow joint HSS scores in both groups were higher than those of the same group before treatment,and the differences were statistically significant(P<0.05);the HSS score of modified elbow joint in experimental group was(69.03±15.88)points,which was higher than(64.43±16.34)points in control group,but there was no statistical significance(P>0.05).The total effective rate of the experimental group was higher than that of the control group,but the difference was not statistically significant(P>0.05).Before treatment,there was no significant difference in the active range of motion(AROM)of elbow joint flexion and extension between the two groups(P>0.05);after treatment,the AROM of elbow joint flexion in the experimental group was(76.17±10.74)°,which was significantly greater than(68.43±14.58)°in the control group,and the AROM of elbow joint extension was(-0.70±1.42)°,which was significantly smaller than(-1.23±2.01)°in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Conventional rehabilitation treatment alone and ESWT combined with conventional rehabilitation treatment can play a better role in the treatment of patients after elbow joint fracture surgery,but ESWT combined with conventional rehabilitation treatment has a more significant effect in improving elbow flexion and extension function of elbow joint.
