摘要
目的分析抗环瓜氨酸肽(CCP)抗体、抗核抗体(ANA)、抗可提取核抗原(ENA)抗体对类风湿关节炎(RA)临床诊断的临床意义。方法选取RA患者124例和对照组649例,用间接免疫荧光法检测ANA、免疫印迹法检测抗ENA抗体、ELISA法检测抗CCP抗体浓度;对抗核抗体,抗ENA抗体,抗CCP抗体的结果和临床资料进行统计学分析。结果RA患者组抗CCP抗体阳性率、ANA阳性率和抗ENA抗体阳性率均明显高于对照组(P<0.01);抗CCP抗体浓度呈非正态分布,RA患者组抗CCP抗体浓度92.15(20.92,203.28)Ru/ml,明显高于对照组(P<0.01);ANA阳性的RA患者抗CCP抗体浓度90.2(23.4,209.58)Ru/ml高于ANA阴性的抗CCP抗体浓度23.1(12.25,56.29)Ru/ml(P<0.01);抗CCP抗体对RA诊断的敏感性为85.48%、特异性为96.15%,而ANA对RA诊断的敏感性为66.94%,特异性为80.27%。结论抗CCP抗体是诊断RA的敏感性、特异性较好的实验室指标,ANA和抗ENA抗体对RA的诊断有一定的参考价值。
Objective To analyze the value of anti-CCP antibody,anti-nuclear antibodies and anti-ENA antibodies in the clinical diagnosis of RA and explore the relationship between anti-CCP antibody concentration and anti-nuclear antibodies in RA patients.To provide reference for the diagnosis and disease judgment of clinical RA.Methods 124 patients with RA and 649 cases with control group were selected.ANA was detected by indirect immunofluorescence,anti-ENA antibody was detected by immunoblotting,and anti-CCP antibody concentration was detected by ELISA.Statistical analysis was performed on the results of the study subjects ANA,anti-ENA antibodies,anti-CCP antibodies and clinical data.Results This study found that the anti-CCP antibody positive rate,ANA positive rate and anti-ENA antibody positive rate of the RA patients group were significantly higher than those of the control group(P<0.01).Anti-CCP antibody concentration is non-normally distributed,and the anti-CCP antibody concentration of RA patients 92.15(20.92,203.28)Ru/ml was significantly higher than that of the control group(P<0.01).The anti-CCP antibody concentration 90.2(23.4,209.58)Ru/ml in the ANA-positive RA patients was higher than the anti-CCP antibody concentration 23.1(12.25,56.29)Ru/ml in the ANA-negative RA patients(P<0.01).The sensitivity of anti-CCP antibody to RA diagnosis was 85.48%,specificity was 96.15%,and sensitivity of ANA to RA diagnosis was 66.94%and specificity was 80.27%.Conclusion As a medical laboratory indicator for the diagnosis of RA,anti-CCP antibody has good sensitivity and specificity.ANA and anti-ENA antibodies have certain reference value for the diagnosis of RA.
