18F-FDG PET/CT显像诊断感染性心内膜炎的Meta分析 被引量:1

Meta-analysis of 18F-FDG PET/CT Imaging in the Diagnosis of Infective Endocarditis

摘要 目的通过Meta分析综合评估18F-FDG PET/CT对于感染性心内膜炎的临床诊断价值。方法在PubMed、Web of Science、Scopus、万方、中国知网、维普等数据库检索利用18F-FDG PET/CT诊断感染性心内膜炎的论文,利用诊断性试验质量评价工具-2评价纳入文献的质量,计算汇总敏感度、特异度、似然比与诊断优势比,绘制汇总操作者受试曲线并计算曲线下面积。结果本次Meta分析共纳入12篇文献合计666例患者,其中包括感染性心内膜炎患者285例,对照组381例。Meta分析结果显示18F-FDG PET/CT诊断感染性心内膜炎的敏感度为0.70,特异度为0.91,阳性似然比为5.32,阴性似然比为0.40,诊断优势比分别是15.44,汇总操作者受试曲线下面积为0.84。结论18F-FDG PET/CT对感染性心内膜炎具有一定的诊断价值,可作为诊断感染性心内膜炎的新方法。 Objective The clinical diagnostic value of 18 F-FDG PET/CT in infective endocarditis was evaluated by meta-analysis.Methods Search for papers on the diagnosis of infective endocarditis by 18 F-FDG PET/CT in Pubmed,Web of Science,Scopus,Wanfang,China knowledge Network,Weipu and other databases.Use diagnostic test quality assessment tool-2 to evaluate the quality of the included literature.The sensitivity,specificity,likelihood ratio and diagnostic advantage ratio were calculated,and the test curve of the summary operator was drawn and the area under the curve was calculated.Results A total of 666 patients were included in this meta-analysis,including 285 patients with infective endocarditis and 381 patients in the control group.The results of meta-analysis showed that the sensitivity,specificity,positive likelihood ratio,negative likelihood ratio,diagnostic advantage ratio and area under the curve of 18 F-FDG PET/CT in the diagnosis of infective endocarditis were 0.70,0.91,5.32,0.40,15.44 and 0.84,respectively.Conclusion 18 F-FDG PET/CT has a certain diagnostic value for infective endocarditis and can be used as a new method for diagnosing infective endocarditis.
