摘要
目的探讨戊酸雌二醇片联合黄体酮对葡萄胎清宫术后子宫内膜修复的效果。方法选取2014年10月至2017年10月攀枝花市第二人民医院诊治的90例行清宫术的葡萄胎患者作为研究对象。按随机数字表法将患者分为两组,每组45例。观察组患者术后接受戊酸雌二醇片联合黄体酮治疗,对照组患者术后不服药。比较两组患者术后阴道出血时间、月经恢复时间以及术后21d子宫内膜厚度,并记录两组患者术后并发症和不良反应发生情况。结果观察组患者和对照组患者术后阴道出血时间分别为(5.76±1.14)d、(7.56±1.36)d,月经恢复时间分别为(28.10±3.08)d、(35.49±3.68)d,术后21d子宫内膜厚度分别为(6.32±1.13)mm、(4.89±1.19)mm,差异具有统计学意义(P<0.05);观察组患者周期性下腹痛、盆腔压痛、闭经发生率均低于对照组患者,差异具有统计学意义(P<0.05);观察组患者发热、腹泻、疲乏、恶心呕吐等发生率与对照组患者相比,差异无统计学意义(P>0.05)。结论对行清宫术治疗的葡萄胎患者采用戊酸雌二醇片联合黄体酮治疗效果显著,能有效缩短术后出血时间,促进月经复潮,利于子宫内膜的修复,降低术后并发症发生率且不会增加用药毒副反应,安全性较高。
Objective To investigate the effects of estradiol valerate tablets combined with progestin on postoperative endometrium repair in hydatidiform mole with complete curettage of uterine cavity. Methods 90 hydatidiform mole patients undergoing complete curettage of uterine cavity Panzhihua Second People′s Hospital from October 2014 to October 2017 were selected, and they were divided into two groups by random number table. Observation group(n=45) was given estradiol valerate tablets combined with progestin after the surgery, while control group(n=45) was not given medicine after the surgery. The postoperative vaginal haemorrhage time, resumption time of menstruation, postoperative 21 d endometrial thickness were compared between the two groups. The postoperative complications and adverse reactions of two groups were recorded. Results The postoperative vaginal haemorrhage time, resumption time of menstruation, and postoperative 21 d endometrial thickness were [(5.76±1.14)d,(28.10±3.08)d,(6.32±1.13)mm] in observation group and [(7.56±1.36)d,(35.49±3.68)d,(4.89±1.19)mm] in control group. There was statistical difference(P<0.05) between the two groups. The incidence of periodic hypogastralgia, pelvic tenderness, amenorrhoea of observation group was lower than that of control group(P<0.05). There was no statistical difference in the incidence of fever, diarrhea, fatigue, nausea and vomiting between the two groups(P>0.05). Conclusions Estradiol valerate tablets combined with progestin in treatment of hydatidiform mole after complete curettage of uterine cavity are effective. It can shorten postoperative haemorrhage time, promote resumption of menstruation, facilitate endometrium repair, decrease the incidence of postoperative complications without increasing toxic-side effects. Thus, it is a safe method.
