地佐辛与舒芬太尼在全身麻醉苏醒期拔管管理中的临床比较

摘要 目的探究地佐辛与舒芬太尼对全身麻醉苏醒期拔管患者的临床应用特点。方法从来该院诊治的2016年8月-2018年9月的腹腔镜胆囊切除术患者中随机抽取64例,采用随机数字法随机分为对照组和观察组,分别采用舒芬太尼治疗和地佐辛治疗,观察两组患者的不良反应、苏醒情况、躁动评分和苏醒期血流动力学情况。结果观察组拔管时间、定向力恢复时间、呼吸恢复时间均低于对照组(P<0.05);观察组恶心呕吐发生率为3.16%低于对照组(χ^2=7.574,P<0.05),呼吸抑制发生率为0.00%低于对照组(χ^2=7.206,P<0.05);观察组在T1时的躁动评分为(1.68±0.29)分低于对照组(t=9.582,P<0.05),T2为(1.63±0.17)分低于对照组(t=10.495,P<0.05),T3为(2.22±0.18)分低于对照组(t=8.935,P<0.05);观察组患者的T1、T2的DBP、SBP和HR水平较为稳定,且低于对照组(P<0.05)。结论地佐辛和舒芬太尼均能防止腹腔镜胆囊切除术患者在全麻苏醒时期出现躁动症状,其中地佐辛更加有效降低苏醒恢复时间,且血液动力学更加稳定,具有更显著的临床疗效。 Objective To investigate the clinical application characteristics of dizocine and sufentanil in patients with extubation during general anesthesia recovery.Methods A total of 64 patients who underwent laparoscopic cholecystectomy from August 2016 to September 2018 in our hospital were randomly selected and randomly divided into control group and observation group.Sufentanil and dizocine was used respectively.The adverse reactions,recovery,agitation score and hemodynamics during the recovery period were observed in the two groups.Results The extubation time,orientation recovery time and respiratory recovery time of the observation group were lower than those of the control group (P<0.05).The incidence of nausea and vomiting in the observation group was 3.16% lower than that of the control group (χ^2=7.574,P<0.05).The inhibition rate was lower than 0.00% in the control group (χ^2=7.206,P<0.05).The agitation score of the observation group at T1 was (1.68±0.29)points,which was lower than that of the control group (t=9.582,P<0.05),and T2 was (1.63±0.17)points were lower than the control group (t=10.495,P<0.05),T3 was (2.22±0.18)points lower than the control group (t=8.935,P<0.05);the observation group T1,T2 of DBP,SBP The HR level was stable and lower than the control group (P<0.05).Conclusion Both dizocine and sufentanil can prevent agitation in patients undergoing laparoscopic cholecystectomy during the general anesthesia period.Dizocine is more effective in reducing recovery time and hemodynamics is more stable,with significant clinical efficacy.
