LED蓝光照射联合静脉营养对低体重高胆红素血症患儿的临床效果 被引量:8

摘要 目的:探讨LED蓝光照射联合静脉营养支持治疗出生低体重合并高胆红素血症患儿的疗效及对患儿相关并发症的影响。方法:选取2016年10月至2018年3月于四川省成都市妇女儿童中心医院分娩出生的103例低体重高胆红素血症患儿,出生体重小于1.5 kg,用随机数字表法将患儿分为对照组与研究组,对照组给于单纯LED蓝光照射治疗,研究组在对照组治疗的基础上加用静脉营养治疗。观察两组患儿生长发育情况,治疗前、后对比血常规检验指标,并对两组患儿症状消失时间进行组间比较。结果:治疗前,两组患儿的头围、体重比较,差异无统计学意义(P>0.05);治疗后,两组患儿的头围、体重均增加,且研究组患儿明显优于对照组(P<0.05)。研究组患儿蓝光照射时间、黄疸消退时间及住院时间均短于对照组(P<0.05)。研究组患儿总胆红素、结合胆红素水平均低于对照组(均P<0.05),两组患儿丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)及甘油三酯(TG)水平比较,差异无统计学意义(P>0.05)。研究组并发症发生率低于对照组(P<0.05)。结论:蓝光照射联合静脉营养治疗能够进一步改善患儿体重及相关生化指标的表达效果,降低并发症发生风险,保障患儿正常生长发育,值得进一步推广研究。 Objective:To investigate the effect of LED blue light combined with intravenous nutrition support on the treatment of low birth weight children with hyperbilirubinemia and the influence of related complications. Methods:103 low-weight and hyperbilirubinemia children born in our hospital from October 2016 to March 2018 were enrolled in the study. Their birth weight was less than 1.5 kg. The children were randomly divided into a control group and a research group. The children in the control group were treated with pure LED blue light, and those in the research group were treated with intravenous nutrition on the basis of the study group. The growth and development were observed before and after treatment. The blood routine test indexes and the disappearance time of symptoms were compared.Results:There was no significant difference in the head circumference and body weight between the two groups before treatment(P>0.05). After treatment, the head circumference and body weight in the study group were significantly higher than those in the control group(P<0.05). The blue light irradiation time, jaundice subsidence time and hospital days in the study group were shorter than those in the control group(P<0.05). The levels of total bilirubin and conjugated bilirubin in the study group were lower than those in the control group(P<0.05). There was no significant difference in ALT, AST and TG levels between the two groups(P>0.05). The incidence of complications in the study group was lower than that in the control group(P<0.05). Conclusion:Blue light irradiation combined with intravenous nutrition therapy could improve the body weight and the expression of related biochemical indicators, reduce the risk of complications and ensure the healthy growth of children with low weight and hyperbilirubinemia.
