不同蓝光及照射方式治疗新生儿高胆红素血症的临床效果比较 被引量:21

摘要 目的比较不同蓝光及照射方式治疗新生儿高胆红素血症的临床效果。方法选择2013年1月至2015年6月咸阳市第一人民医院收治的高胆红素血症新生儿160例,按照照射光源不同分为冷光源蓝光组(80例)和传统蓝光组(80例),根据照射方式不同冷光源蓝光组分为冷光源持续照射组(A组,40例)和冷光源间歇照射组(B组,40例),传统蓝光组分为传统蓝光持续照射组(C组,40例)和传统蓝光间歇照射组(D组,40例),治疗时间均为72 h。比较各组的疗效和不良反应发生情况。结果 A组和B组总有效率分别为97.5%(39/40)、97.5%(39/40),均高于C组[78.5%(31/40)]和D组[72.5%(29/40)],差异有统计学意义(P<0.05)。A和B组总不良反应发生率均为7.5%(3/40),均低于C组[55.0%(22/40)]、D组[35.0%(14/40)],差异有统计学意义(P<0.05)。治疗后3 d,A组和B组ABR异常率分别为12.5%(5/40)、7.5%(3/40),均低于C组[32.5%(13/40)]、D组[35.0%(14/40)],差异有统计学意义(P<0.05)。结论采用冷光源蓝光照射治疗新生儿高胆红素血症临床效果明显,可以有效降低新生儿听力损害。 Objective To compare clinical effect of different blue and irradiation manners in treatment of neonatal hyperbilirubincmia. Methods Total of 160 cases of hypcrbilirubinemia in neonates admitted to Xianyang First People's Hospital from Jan. 2013 to Jun. 2015 were included in the study,and were divided into group A with 40 cases receiving cold blue light continuous irradiation, group B with 40 cases receiving cold blue light intermittent irradiation, group C with 40 eases receiving traditional blue continuous irradia- tion ,and group D with 40 cases receiving traditional blue light intermittent irradiation. The treatment time was 72 h for all patients, and comparative analysis of curative effect and adverse reaction of each group was done. Results The total effective rate of group A and group B were 97.5% (39/40) ,97.5% (39/40) ,higher than group C[78.5% (31/40) ] and group D[72.5% (29/40) ] ,the differences were statistically significant ( P 〈0. 05 ). The total adverse reaction rate of group A and B were 7.5 % (3/40) ,7.5 % (3/40) ,lower than group C [55.0% (22/40) ] and group D[35.0% (14/40) ] ,the differences were statistically significant( P 〈 0.05 ). Three days after treatment, ABR abnormal rate of group A and B were 12. 5% ( 5/40 ), 7.5% (3/40) , lower than group C [ 32.5% ( 13/40 ) ] , group D [ 35.0% ( 14/40 ) ] , the differences were statistically significant(P 〈 0. 05 ). Conclusion The treatment of the cold source blue light irradiation for neonatal hyperbilirebinemia has obvious clinical effect, which can effectively reduce the neonatal hearing impairment.
