摘要
目的探究Fogarty球囊导管取栓术与置管溶栓术治疗急性下肢动脉栓塞的临床效果。方法回顾性分析本院2016年1月至2018年12月收治的68例急性下肢动脉栓塞患者的临床资料,按治疗方法的不同将其分为Fogarty球囊组(Fogarty球囊导管取栓术)和置管溶栓组(置管溶栓术),各34例。比较两组患者的治疗效果、手术前后踝-肱指数(ABI)和足部皮温的变化情况以及随访情况。结果 Fogarty球囊组患者的治疗总有效率为76.47%,低于置管溶栓组患者的94.12%(P<0.05);手术后,两组患者ABI和足部皮温均显著升高,且置管溶栓组高于Fogarty球囊组(P<0.05);置管溶栓组患者的再栓塞率显著低于Fogarty球囊组(P<0.05),但两组的截肢率无显著差异(P>0.05)。结论相较于Fogarty球囊导管取栓术而言,置管溶栓术治疗急性下肢动脉栓塞的临床有效率更高,患者足部皮温和动脉压力恢复情况更好,同时再栓塞率和截肢率较低,整体治疗效果更好。
Objective To investigate the clinical effects of Fogarty catheters embolectomy and catheter directed thrombolysis on acute lower limb arterial embolism. Methods The clinical data of 68 patients with acute lower limb arterial embolism admitted in our hospital from January 2016 to December 2018 were retrospectively analyzed. According to different treatment methods, the patients were divided into Fogarty balloon group(Fogarty catheters embolectomy) and catheter thrombolysis group(catheter directed thrombolysis), with 34 cases in each group. The therapeutic effects, anklebrachial index(ABI) and foot skin temperature before and after operation, follow-up were compared between the two groups. Results The total effective rate of treatment in the Fogarty balloon group was 76.47%, which was lower than94.12% of the catheter thrombolysis group(P<0.05). After operation, the ABI and foot skin temperature of the two groups increased significantly, and those in the catheter thrombolysis group were higher than the Fogarty balloon group(P<0.05).The reembolization rate in the catheter thrombolysis group was significantly lower than that in the Fogarty balloon group(P<0.05), but there was no significant difference in amputation rate between the two groups(P>0.05). Conclusion Compared with Fogarty catheters embolectomy, catheter directed thrombolysis is more effective in the treatment of acute lower limb arterial embolism, with better recovery of skin temperature and arterial pressure, lower reembolization rate and amputation rate, and better overall therapeutic effect.
