摘要
目的评价精子顶体酶活性和精子-透明质酸结合率在精子受精能力中的作用,查找不明原因性不育症的病因。方法选择2015年7月至2017年8月安阳市妇幼保健院诊治的符合不明原因性不育症的385例患者为研究对象。在进行精子动态学(密度、活率、前向运动率)和形态学检查的基础上,检测精子顶体酶活性和精子-透明质酸结合率,比较各项参数的相关性,对其中162例患者在知情同意情况下行精液优化和人工授精治疗,随访治疗结果。结果精子顶体酶活性与精子动态学(密度、活率、前向运动率)和形态学等各参数呈正相关(P<0.05),精子顶体酶活性和精子-透明质酸结合率均正常者的人工授精妊娠率显著高于均异常者(P<0.05)。结论精子顶体酶活性和精子-透明质酸结合率异常是不明原因性不育症的潜在病因,对这些患者检测精子顶体酶活性尤为重要。
Objective To evaluate the role of sperm acrosin activity and sperm-hyaluronic acid binding rate in sperm fertilization ability, in order to find the cause of unexplained infertility. Methods A total of 385 patients with unexplained infertility from July 2015 to August 2017 visiting in Anyang Women and Children Health Hospital were screened for sperm dynamics(concentration, survival rate, forward movement rate) and morphological examination. The sperm acrosin activity and sperm-hyaluronic acid binding rate were measured, and the correlations between the parameters were compared. 162 patients in the 385 cases were treated with semen optimization and artificial insemination in the context of informed consent, and the treatment outcomes were investigated. Results The sperm acrosin activity was positively correlated with sperm dynamics(concentration, viability, forward movement rate) and morphological parameters(P<0.05). The pregnancy rate of artificial insemination with normal sperm acrosin activity and sperm-hyaluronic acid binding rate was significantly higher than that of abnormal ones(P<0.05). Conclusions The abnormality of sperm acrosin activity and sperm-hyaluronic acid binding rate is a potential cause of unexplained infertility. Therefore, it is particularly important to detect sperm acrosin activity in these patients.
